About this file

This is a Quote Response Guide that provides instructions and formatting requirements for contractors submitting proposals to NASA Stennis Space Center for Occupational Health Medical Director and Employee Assistance Program (EAP) Coordinator support services.

The guide establishes a four-volume submission structure. Volume I requires executive information including contractor details, acknowledgment of RFQ amendments, primary and authorized points of contact, UEI and CAGE codes, and any exceptions to terms and conditions with supporting rationale. Volume II mandates submission of past performance references (two required, maximum two pages each) demonstrating relevant experience with government or commercial agencies, including contract details, period of performance, contract value, work location, current status, and relevance to the performance work statement. Volume III addresses technical evaluation across three criteria: corporate experience performing occupational health services compliant with federal, state, and NASA requirements (including NPRs 1800.1 and 1850.1), credentialing and compliance management, clinical program development (immunizations, medication management, infection control, CDC/OSHA compliance), EAP clinical services delivery, and federal mental health standards compliance; key personnel qualifications for the Medical Director position (limited to two pages, no resumes); and capability/understanding narrative (limited to three pages) demonstrating comprehension of occupational health scope, regulatory frameworks (NPRs, OSHA, CDC, HIPAA, Privacy Act, 5 CFR 792, 42 CFR Part 2), service delivery approach, quality assurance processes, and mitigation strategies for performance challenges. Volume IV requires use of a provided Excel pricing template with no deviation from format permitted.
