About this file
This is a Quality Assurance Surveillance Plan (QASP) for a Defense Logistics Agency contract to supply up to 97.7 metric tons each of Ti CP2 and Ti 6-6-2 titanium within a two-year period, with delivery to the DLA Strategic Materials Storage Facility at Hawthorne Army Depot in Nevada.
The QASP outlines detailed quality control requirements including: sampling and testing protocols through an independent third-party laboratory that must be ISO 17025 certified, NADCAP accredited, ASME-NQA-1 compliant, and ITAR compliant; specific marking/labeling requirements for ingots; documentation requirements including Certificates of Analysis and Safety Data Sheets; and inspection procedures where the COR conducts both 100% and random inspections with pass/fail ratings. Key timing requirements include 30 business days notice before processing/sampling/testing, 20 business days notice before deliveries, and 5-day turnaround for laboratory analyses. The contractor must receive written pre-approval for all shipping from the COR and cannot invoice until each ingot is accepted in writing.

Perform the following specific service(s)/tasks;
1) The Contractor shall perform all service(s)/tasks to satisfy the above listed “Required Service/Task” and any additional service(s)/tasks required by the SOW.
Primary Methods of Surveillance: 100% and Random inspections.
Frequency of Surveillance: The COR will perform inspections. Based on observed defects in contractor performance, subsequent inspections may either be reduced or increased. At a minimum, the COR will complete a “Government Inspection Report” monthly. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report”).
Acceptable Performance Level (APL): All service(s)/tasks shall be performed at or above the Acceptable Performance Levels (APL’s) listed in the Performance Requirements Summary Table (PRST) of this SOW.
Method of Surveillance: The method of surveillance will be random unless listed differently in the Performance Requirements Summary Table (PRST) of this SOW.
Surveillance Procedures: 100% inspections or Random inspections will be performed on scheduled service(s)/tasks unless listed differently in the Performance Requirements Summary Table (PRST) of this SOW. There are no special procedures to be followed to initiate unscheduled inspections.
Evaluation Procedures: The COR will observe the selected service(s)/tasks at any time they determine a need to inspect a service/task or notices a deficiency. Results of each inspection will be documented on a “Government Inspection Report”. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report). A “pass”, “fail” or “N/A” rating will be assigned to each service/task inspected based on the Performance Criteria listed below. A brief description of the inspection and observed defects will be recorded. A “fail” rating may require contractor re-performance of service(s)/tasks. The COR will ascertain whether documented defects were corrected, and processes implemented to ensure re-occurrence of the defect is prevented. An “N/A” rating will be assigned when no work was required to be performed on a specific service/task during that rating period.
Performance Criteria:
1) Each “Required Service/Task” requirement may contain several services/tasks. Each requirement will be evaluated based on an individual service/task.
2) At a minimum, the COR will inspect all service(s)/tasks listed in the Performance Requirements Summary Table (PRST) of this SOW at the frequency listed in the PRST.
3) A “pass” rating will be assigned to each service/task inspected that met or exceeded the established Acceptable Performance Level (APL) for each “Required Service/Task” requirement evaluated.
4) A “fail” rating will be assigned to each service/task inspected that fell below the established Acceptable Performance Level (APL) for each “Required Service/Task” requirement evaluated.
When a “fail” rating is assigned, all observed defects must be documented on a “Government Inspection Report”. (See Attachment 4 to this SOW for a copy of the “Government Inspection Report”).
QASP_Ti_17Jan25 Page 4 of 4
5) An “N/A” rating will be assigned when no work was required to be performed on a specific service/task during that rating period.
Analysis of Results:
1) The COR will notify the Contractor within 48 hours each time a “fail” rating is assigned and discuss the basis for the rating.
2) The COR will solicit the Contractor’s proposed corrective action(s) to preclude re-occurrence of the defective service(s)/task performance.
3) Based upon the Contractor’s performance, the COR may recommend corrective action(s).
Subsequent inspections may be performed more frequently if defects continue.
4) The COR will document the results of each inspection using a “Government Inspection Report”.
(See Attachment 4 to this SOW for a copy of the “Government Inspection Report”). The COR will complete a “Government Inspection Report” monthly at a minimum. The COR will upload copies of all completed “Government Inspection Reports” into the SPM Application within the Procurement Integrated Enterprise Environment (PIEE), the web-based system.
Combatting Trafficking in Persons: The COR will monitor the vendor compliance with Combatting Trafficking in Persons (CTIP) clause in accordance with DFARS PGI 222.1703(4), such that noncompliance with 52.222-50 is documented to the KO, utilizing and annually completing the CTIP checklist at: DoD CTIP website.
