About this file

This is a Government Inspection Report template that outlines the quality control and inspection requirements for a titanium ingot acquisition contract. The report covers inspection standards across multiple Statement of Work (SOW) sections including packaging, testing, labeling, and delivery requirements.

The inspection criteria require 100% compliance across all performance areas, with pass/fail ratings assigned by the Contracting Officer Representative (COR). Key requirements include proper packaging in approved drums with polyethylene liners, accurate labeling including GHS labels, submission of Certificates of Analysis, advance notification of packaging operations, and scheduling deliveries 20 business days in advance. The document relates to solicitation SP800025Q0002 for up to 174 metric tons of titanium ingots (Ti-CP-2 and Ti-CP-6-6-2) to be delivered to Hawthorne, Nevada, with an estimated value of $5 million and a 24-month performance period.

Titanium Ingots Acquisition 2025

Contract Number:Dates of Inspection:
Contractor:COR:

The COR will observe the selected service(s) at any time they determine a need to inspect a service or notices a deficiency. Results of each inspection will be documented on a “Government Inspection Report”. A “pass”, “fail” or “N/A” rating will be assigned to each service inspected. A "pass" rating will be assigned when the Contractor performs a rated service at or above the listed "Acceptable Performance Level (APL)". A “fail” rating will be assigned when the Contractor performs a rated service below the listed "Acceptable Performance Level (APL)". A brief description of the inspection and observed defects will be recorded. The COR will ascertain whether documented deficiencies were corrected and processes implemented to ensure re-occurrence of the deficiency is prevented. A “N/A” rating will be assigned when no work was required to be performed on a specific service. The COR will upload copies of all completed “Government Inspection Reports” into the SPM Application within the Procurement Integrated Enterprise Environment (PIEE), the web based system.
