About this file
This is a Past and Present Performance Information Form (PPIF) template used to collect performance evaluations of contractors on federal contracts.
The form is organized into four main sections. Section A requires general information about the contractor, including company name, contact details, address, and whether the contractor performed as a prime contractor, sub-contractor, or key personnel. It also requests project title, contract number, dollar amount, and performance dates. Section B collects information about the evaluator completing the form, including name, title, company affiliation, and contact information. Section C provides submission instructions directing completed forms to Sherry Platt at sherry.platt.1@us.af.mil or (701) 723-4180. Section D contains the performance evaluation matrix using a six-point rating scale (E=Exceptional, G=Very Good, S=Satisfactory, M=Marginal, U=Unsatisfactory, N=Neutral) with detailed definitions for each rating level. Evaluators must rate contractor performance across 24 criteria addressing management and supervision, personnel qualifications, quality control, schedule adherence, problem resolution, and overall performance. The form requires narrative explanations for any Marginal or Unsatisfactory ratings. Additional yes/no questions address cure notices, willingness to award future contracts, and CCASS ratings. A remarks section allows for supplemental comments. The form is designated SOURCE SELECTION SENSITIVE under FAR 3.104 when completed.
