About this file
This is a Pre-Construction Risk Assessment document (Rev 6) for federal construction project 668-22-105, titled "Upgrade Mechanical Systems AHU 9&15." The project is planned to commence on 12/31/2025 with an estimated duration of one year and involves work at Building 1, Floor 2 Imaging, Floor 3 Mech Penthouse, and Floor 5 ICU.
The assessment covers nine risk categories requiring evaluation and sign-off by designated facility representatives. Fire and life safety impacts address egress routes, exit stairs, fire compartments, alarm systems, suppression systems, smoke partitions, and emergency access, with multiple items identified as requiring Interior Life Safety Management (ILSM). Safety considerations include confined space work permits and hot work permits, with notation that crane placement may impact Fire Department access and hot work permits must be submitted to Safety prior to performance. Physical security impacts note temporary closure of a loop due to crane placement near the ambulance entrance. Construction activity section identifies anticipated short-duration HVAC outages coordinated for after-hours or weekends, crane operations, and potential traffic disruptions. Environmental review indicates no NEPA assessment above categorical exclusion is required. Hazardous materials assessment documents that an asbestos-containing material (ACM) survey identified ACM flooring in the A524 Mechanical Room, requiring a qualified ACM abatement team for remediation prior to construction. Infection Control Risk Assessment (ICRA) has been completed. Patient safety considerations include potential reduction of available beds, required Crane Lift Plan submission, and potential minor patient flow impacts from crane and construction fence placement. All required sections have been signed and dated by respective facility specialists and safety officers.
