About this file
This is a Crematorium Inspection Checklist template used to evaluate funeral establishments and crematoria for their eligibility to provide contractual mortuary services to the federal government.
The checklist assesses five primary categories of facility compliance: (1) Receiving/Sheltering Area—evaluating adequate size, equipment for remains transfer, identification instruments, capacity for at least two remains, ventilation, and cleanliness; (2) Retort—confirming operational burners and safe operation within manufacturer specifications; (3) Processing of Cremated Remains—verifying that all remains are swept from the retort after each cremation, foreign objects are removed and properly disposed of, mechanical processing equipment such as a cremulator is available, and remains can be reduced to particles no larger than 1/8 inch; and (4) Standard Criteria—confirming compliance with all applicable specifications, laws, and statutes, that the crematorium is licensed solely for human remains, and that competent professional staff is available. The form includes yes/no checkboxes for each criterion and concludes with certification signature lines for the Contracting Officer and Mortuary Officer/Other Officer to determine whether the crematorium is acceptable to provide contractual mortuary services.
