ATTACHMENT 4 – FA4600-21-Q-0009 - Site Visit Instructions - Offutt AFB Grounds Maintenance The site visit will be held on February 1st. There will only be one site visit. In order to attend the site visit the following shall be completed by interested parties:
1. Complete and submit Attachment 5 – Vetting Request for Site Visit.
2. Take a picture of each driver’s license (or other ID) used to fill out the vetting form for all people listed on the form.
3. Email both the completed form and pictures of the IDs to Mr. Benjamin Duncan at benjamin.duncan.7@us.af.mil with a notification that your company would like to attend the site visit. All requests to attend the site visit must be received no later than 4:00 pm CDT on Monday, January 25, 2021.
4. Interested parties are responsible for ensuring the request is received by Mr. Duncan. It will take a week to process individual background checks for access to Offutt AFB.
5. Attendees will not be notified if a background check comes back negative. Negative background checks will result in denial of access to Offutt AFB.
