About this file

This file is a Past Performance Questionnaire (PPQ) template and submission instructions for the USCGC KIMBALL FY26 Dockside repair solicitation (70Z08526QLREP0013). The document establishes the evaluation framework for assessing offeror past performance on relevant projects as part of Factor II evaluation criteria.

Offerors must submit either completed CPARS evaluations for relevant past projects or have customers complete the PPQ form included in the solicitation. Customers must rate contractor performance across three categories—Quality of Product or Service, Timeliness of Performance, and Business Relations—using a 0-5 scale (Unsatisfactory through Outstanding). Completed questionnaires must be submitted directly by the customer via email to Wayne Gutman (Wayne.M.Gutman@uscg.mil) with a copy to Andrew Jacobs (Andrew.G.Jacobs@uscg.mil), with the solicitation number and company name referenced in the subject line. All questionnaires must arrive by the solicitation close date; late submissions will not be reviewed. The PPQ requires offerors to complete sections identifying their firm name and performance period, with customers providing detailed evaluation ratings, comments, contract details, and signature verification. Rating guidelines specify evaluation criteria including compliance, accuracy, technical excellence, milestone achievement, timeliness, delivery schedules, management effectiveness, and contractor responsiveness.
