MEMORANDUM FOR QUESTIONNAIRE EVALUATORS
FROM: 766th Enterprise Sourcing Squadron/PKI 560 Central Avenue JBPH-H, Hawaii 96860
SUBJECT: Request for Past Performance Questionnaire Responses
1. The 766th Enterprise Sourcing Squadron is in the process of selecting a contractor for Visual Information Services at Daniel K. Inouye Asia Pacific Center For Security Studies (DKI APCSS).
2. One of the considerations in proposal evaluation is the verification of the offeror’s past and present performance. We value and depend upon information received from agencies and clients such as yourselves, who have first hand customer experience with an offeror’s performance of services. I am asking that you help us evaluate offerors that you have used, by sharing your assessment of how these offerors have performed on contracts which have served you.
3. Our areas of interest in the offeror are summarized in the enclosed questionnaire. Our schedule is extremely tight and we need to receive your written response no later than 8 calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
4. DO NOT RETURN QUESTIONNAIRE TO OFFEROR/CONTRACTOR! Once the questionnaire is filled out, it becomes source selection sensitive and needs to be handled accordingly. Although this questionnaire was distributed to you by the offeror/contractor, it is important that you return the completed questionnaire to the Government Contracting Officers at the following address.
