About this file
Attachment 3 - Past Project Form is a standard federal contract document template designed for contractors to provide detailed historical project information when responding to a solicitation. The form requires contractors to document comprehensive details about previous contracts, including agency/organization name, contract number, project award date, total project price, performance period, geographic location, and performance contacts.
The template includes sections for contractors to describe the scope, magnitude, and complexity of past work, comparing it to the current solicitation (70RFP325QE3000018) for Canine Explosive Scent Kits from the Department of Homeland Security Office of Procurement Operations. Contractors must also disclose any subcontracting plans, performance issues, or Department of Labor investigations, and are required to indicate whether a finalized Contractor Performance Assessment Report (CPARS) exists for the referenced contract. The form serves as a mechanism for the government to evaluate a contractor's past performance and capability to execute the proposed work.
