About this file

This is a Past Performance Questionnaire (PPQ) template for RFP 36C24726R0069, titled "Correct Critical Access and ADA Deficiencies B61." The document is designed to collect performance evaluations from previous clients of contractors bidding on this solicitation.

The PPQ consists of three sections. Section I requires offerors to provide identification information for the organization being evaluated, including the reference organization name and address, contract number, project description, contract/task order amount, performance period, and contractor role (prime or subcontractor). Section II contains fourteen evaluation criteria that reference clients must rate on a five-point numerical scale: 5 (Superior Performance), 4 (Very Good Performance), 3 (Satisfactory Performance), 2 (Marginal Performance), 1 (Unacceptable Performance), or N/A (Not Applicable). The evaluation criteria address compliance with contract requirements, on-site management effectiveness, personnel appropriateness, documentation timeliness, schedule adherence, safety responsiveness, subcontractor coordination, customer relations, problem-solving cooperativeness, financial capability, responsiveness to concerns and cure notices, warranty notifications, overall performance, and a yes/no recommendation for future work. Section III provides space for evaluators to provide narrative comments explaining any low ratings. Evaluators must provide their signature, title, phone number, and email address. Completed questionnaires are to be submitted directly to Cayley.blommers@va.gov, and offerors are responsible for ensuring accurate and verifiable contact information for their references.
