About this file

This is a Past Performance Questionnaire used by the 628th Contracting Squadron at Joint Base Charleston, South Carolina to evaluate contractor performance for a competitive selection process. The questionnaire is designed to solicit feedback from references regarding a vendor's prior experience and suitability for performing temporary event rental services for the 2026 Joint Base Charleston Air Show.

The scope of work includes complete delivery, professional setup, and timely teardown of tents, tables, and chairs on the installation. Respondents are instructed to complete the questionnaire by 4:00 PM EST on March 5, 2026, and submit responses to Contract Specialist Leo Lachapelle (leo.lachapelle.2@us.af.mil) and Matthew Michel (matthew.michel@us.af.mil). The questionnaire requests respondent and vendor contact information, contract details including type, performance period, annual and total contract dollar values, and assessment of contractor performance across four key evaluation areas: ability to meet work requirements and quality standards, timely and budget-compliant order completion, responsiveness to quality issues and deficiencies, and communication responsiveness with the Government. Evaluators select ratings using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) and must provide explanations for any ratings other than Satisfactory. The questionnaire also includes Yes/No questions regarding whether the evaluator would award another contract to the firm and whether the contract was terminated for default, convenience, or cause, with required explanations for negative responses.
