About this file

This is a Past Performance Assessment Questionnaire issued by the U.S. Army Mission and Installation Contracting Command (MICC) at Fort Gordon, Georgia in support of RFP W91249-26-RA006 for a Joint Targeting Analyst (JTA) Training Solution Developer (TSD) contract. The questionnaire is completed by past customers or points of contact to evaluate an offeror's historical performance on same or similar federal, state, local government, or commercial contracts comparable to NAICS 611430. Completed questionnaires must be submitted to Contracting Officer Shecoriya Byrd and Contract Specialist Cedric Ellis by March 13, 2026. The assessment uses a four-tier rating system: Substantial Confidence (exceeds requirements with minimal issues), Satisfactory Confidence (meets requirements with some minor issues), Limited Confidence (does not meet some requirements with serious problems), and No Confidence (does not meet most requirements with ineffective corrective actions).

The questionnaire evaluates offeror performance across five key areas: compliance of products, services, documents, and deliverables to specification and workmanship standards; effectiveness of project management including subcontractor control; timeliness of performance for services and product deliverables; commitment to customer satisfaction and business-like concern for customer interests; and general comments on relevant performance information. Part I requires the offeror to identify the contract being referenced, including contractor name, program identification, contract number, contract type, award date, completion date, nature of effort, and total contract value. Part II is completed by the point of contact respondent and requires a rating with written rationale for each evaluation criterion. The questionnaire also requests identification of other relevant past efforts and contact information for additional references.
