About this file

This is a Patient Safety Alert Addendum issued by the Veterans Health Administration (VHA) on September 3, 2014, addressing safety concerns with ceiling-mounted patient lift systems.

The alert documents multiple incidents of ceiling-mounted patient lift failures across VA medical centers, including lifts collapsing during patient transfers, units falling off rails, and bracket failures. Root causes identified include improper installation and maintenance, missing or inadequately secured components (such as end stops, snap clips, and docking gates), and incomplete post-installation inspections. The alert replaces the original Patient Safety Alert AL10-07 issued in March 2010 and establishes updated requirements for installation, maintenance, and inspection procedures.

The alert mandates four primary actions with specific deadlines: (1) by July 30, 2014, Medical Center Directors must notify key personnel of the alert; (2) by December 5, 2014, Managers responsible for ceiling-mounted patient lifts must verify completion of "After Installation Checklists" and implement an "Installation or Relocation Checklist" process for all new installations or relocations; (3) by December 12, 2014, service managers must establish a process ensuring the "Corrective and Preventive Maintenance Checklist" is completed for all preventive and corrective maintenance procedures; and (4) by December 19, 2014, the Patient Safety Manager must document implementation on the VHA Alerts and Recalls Web site. The alert includes additional resources such as installation/relocation checklists, maintenance checklists, hazard summaries, and a ceiling lift hazards mitigation presentation.
