Attachment 1- VA Patient Lift List -203.xlsx

This is an inventory list of patient lift equipment located across multiple VA healthcare facilities within the Greater Los Angeles VA Healthcare System. The spreadsheet provides a comprehensive catalog of 203 Guldmann lifts distributed across four primary locations: West Los Angeles (WLA) with 152 units, Los Angeles Ambulatory Care Center (LAACC) with 1 unit, Southeast Los Angeles Care Center (SACC) with 47 units, and Venice (VEN) with 3 units. The lifts are categorized by model type, including the WLA 500 series (36 units in standard locations, 24 units in ICU settings), WLA 213 series (30 units), and WLA 215 series (62 units), with each unit assigned a unique equipment number and serial number for tracking and maintenance purposes.

Each lift entry includes specific location identifiers and serial numbers to facilitate inventory management and maintenance scheduling in connection with the annual inspection, preventative maintenance, and corrective maintenance services outlined in the related solicitation 36C26226Q0801. The detailed breakdown by facility and model type enables contractors to understand the scope of equipment requiring service across the VA healthcare system and to develop comprehensive maintenance plans tailored to the different lift models and their deployment locations. This inventory serves as the baseline for the service requirements and establishes the universe of equipment that will require ongoing maintenance and inspection support.
