About this file

This is a scoring matrix template for offeror self-evaluation in response to a 161kV Transformer IDIQ solicitation for the Department of the Air Force Materiel Command Test Center. Offerors must complete a self-scoring assessment across five primary evaluation categories with a maximum total of 5,000 points.

The first three categories evaluate work samples in Repair (Category 3.2.2.1), Replace (Category 3.2.2.2), and Spare (Category 3.2.2.3) services, with seven, six, and three subcategories respectively, each worth up to 5 points based on experience level. These categories include specialized transformer work such as complete on-site refurbishment, winding repair, load tap changer rebuild, cooling system refurbishment, tank and structural repair, coating application, and lightning arrestor replacement for transformers rated ≥161kV and ≥30MVA. The combined self-score points across these three categories cannot exceed 20 points total. Category 3.2.2.4 (Project Management, Development, and Design) is scored based on total dollar value of 161kV transformer projects supporting DoD installations, with a maximum of 60 points. Category 3.2.2.5 (CPARS/CPARS Equivalent Data Submission) allows up to five project submissions that generate bonus points calculated as a percentage based on ratings in Quality Management, Cost Control, Small Business Subcontracting, Regulatory Compliance, and Schedule; if offerors submit at least one CPARS entry for each of the first three categories, bonus points are doubled. The matrix enforces constraints that teaming subcontractor work samples and CPARS submissions cannot exceed prime contractor submissions, and all entries must be documented in designated yellow cells with supporting work sample documentation.

Instructions

Instructions
1. Only input data into the yellow cells of the Self Score Sheet tab.
2. Please insert Offeror's name on Row 4.
3. Offeror shall fill in Column D "Offeror Self-Score Points: Work performed by Prime" and Column E "Offeror Self-Score Points: Work performed by Teaming Subcontractor" for the first 5 Categories, 3.2.2.1-3.2.2.3.
4. Sum of Work Samples submitted for each Subcategory cannot break the max allowable points (Column F must be less than or equal to Column C for each subcategory).
5. An Offeror may account for a sum of 20 points across the first three Categories: 3.2.2.1 Repair, 3.2.2.2 Replace, and 3.2.2.3 Spares. Please note that points are referencing the value placed by the Offeror in columns D&E, not the score calculated from those entries.
6. The total number of Work Samples submitted on behalf of a Teaming Subcontractor must be less than or equal to the number of Work Samples submitted for work performed by the Prime (Cell E29 must be less than or equal to Cell D29).
7. All Offerors are to fill in the Project Management, Development, and Design Category (3.2.2.4) based off the projects/work samples submitted for sections 3.2.2.1-3.2.2.3.
8. All Offerors are encouraged to fill out the CPARS/CPARS Equivalent Data Submission Category (3.2.2.5), columns A-I, based off the projects/work samples submitted from Categories 3.2.2.1-3.2.2.3.
9. Offerors are allotted a maximum of five CPARS/CPARS Equivalent submissions (3.2.2.5). It is up to the Offeror to determine best score feasible with the available five submissions. Note that the number of subcontractor CPAR submissions must be less than or equal to the number of CPARs submitted for the Prime.
10. Points allotted from CPARS/CPARS Equivalent data are considered a bonus score; each entry will produce a calculated percentage bonus based off of the specific Subcategory it is associated with and the Offeror's ratings.
7. All Offerors are to fill in the Project Management, Development, and Design Category (3.2.2.4) based off the projects/work samples submitted for sections 3.2.2.1-3.2.2.3.
8. All Offerors are encouraged to fill out the CPARS/CPARS Equivalent Data Submission Category (3.2.2.4), columns A-I, based off the projects/work samples submitted from Categories 3.2.2.1-3.2.2.3.
11. If an Offeror is able to submit at least one CPARS/CPARS Equivalent response for each of the first three Categories (3.2.2.1 Repair, 3.2.2.2 Replace, 3.2.2.3 Spare), then the Offeror will receive a double bonus, doubling the points received for all CPAR submissions.
