About this file

This document is a Proposal/Proposer Certification Sheet, a standard form for a state or local government procurement process. The form requires potential bidders to provide comprehensive business information when submitting a proposal, including organization type (sole proprietorship, partnership, or corporation), federal and state identification numbers, and applicable business certifications. The certification sheet must be signed, dated, and submitted with original signatures in a sealed envelope along with all required attachments.

The form includes specific sections for businesses to indicate their small business or disabled veteran business enterprise status with the California Department of General Services, Office of Small Business Certification and Resources (OSBCR). Proposers must check the appropriate boxes and provide certification numbers if applicable, with a requirement to include a copy of their certification if they qualify. The document emphasizes that an unsigned certification sheet may result in proposal rejection, underscoring the importance of complete and accurate submission of this critical procurement documentation.
