ATTACHMENT 17
Small Business Participation Commitment Document and Progress Report The contractor’s SBPCD shall be incorporated into the contract, and the contractor shall submit annual reports on the commitment document.
Small Business Participation Progress Report shall be in contractor format and include:
(1) Name of the small business subcontractor(s), whether the subcontractor(s) is:
a. Small Business;
b. Small Disadvantaged Business (SDB)
c. Woman-Owned Small Business (WOSB)
d. Veteran Owned Small Business (VOSB)
e. Service Disabled Veteran Owned Small Business (SDVOSB)
f. Historically Underutilized Business Zone (HUBZone) Small Business
(2) The contractor shall briefly describe the subcontractor(s) efforts performed.
(3) The contractor shall report actual achievements relative to proposed small business utilization in performance of the contract (including subcontracting dollars/percentages),
(4) The contractor shall provide explanation for any dollar/percentage shortfalls than what was proposed, and provide an approach for how the Offeror intends to meet the proposed small business participation percentage.
Any changes made to the proposed listing of SBPCD vendors shall be submitted for approval to the Contracting Officer prior to any substitution or removal of vendors.
Failure to meet proposed Small Business Participation / minimum quantitative requirements (MQR) for small business and small business socio-economic categories shall be considered a major nonconformance and may result in the following actions taken by the contracting officer: Unsatisfactory rating to small business subcontracting and Management or business relations Contract Performance Assessment Reporting System (CPAR) evaluation factors (See FAR 42.1503(b)(2), Table 42-2).
