About this file

This is a sample template document for a Past Performance Evaluation Questionnaire and cover letter used by federal contracting offices to solicit performance information from agencies with prior experience with potential contractors.

The questionnaire template is designed to be sent to past customers or contracting partners of companies bidding on federal contracts. It requests factual and accurate information about contractor performance within the past three years across six evaluation categories: compliance with specification requirements and workmanship standards, effectiveness of project management and subcontractor control, timeliness of performance, cost forecasting and control effectiveness (for cost-reimbursement contracts only), commitment to customer satisfaction, and general performance comments. Respondents rate performance using standardized adjectival categories—Outstanding, Good, Acceptable, Marginal, or Unsatisfactory—with mandatory explanations required for Outstanding, Marginal, and Unsatisfactory ratings. The cover letter instructs respondents to return completed questionnaires within a specified timeframe (recommended to align with the solicitation closing date) via email or fax to facilitate timely source selection analysis. The template includes sections for contractor identification, offeror representative information, evaluation responses with comment fields, identification of other relevant past efforts, and respondent contact details. This document serves as a standardized tool for past performance verification during federal contract award evaluation processes and becomes part of official source selection records.
