About this file
This is an Iran Contracting Act Verification Form (DGS PD 3) issued by the California Department of General Services Procurement Division for state contracts valued at $1,000,000 or more. The form applies to RFP C5612999, an IP Mail Scanning Solution procurement for the State of California. Prior to bidding, vendors must either certify compliance with California Public Contract Code sections 2202-2208 by confirming they are not listed on the DGS Iran Contracting Act List and do not extend $20,000,000 or more in credit to persons providing goods or services in Iran's energy sector, or demonstrate an approved exemption from the certification requirement on a case-by-case basis.
The form provides two options for vendor compliance. Under Option 1, vendors certify their eligibility by providing their name, Federal ID Number, and authorized signature. Under Option 2, vendors with an approved exemption from the Iran Contracting Act requirements must submit documentation demonstrating exemption approval along with required vendor information and authorization. California law establishes significant penalties for false certifications, including civil penalties of $250,000 or twice the contract amount, contract termination, and three-year ineligibility to bid on future state contracts.
