The form prohibits procurement of equipment, systems, or services that use covered telecommunications equipment as substantial or essential components or critical technology. Offerors should reference the excluded parties list in the System for Award Management (SAM) and Attachment No. 3 of the solicitation for submission instructions. This representation must be completed and submitted with the quote unless the offeror has already provided equivalent representations in SAM through FAR 52.204-26 or in FAR 52.212-3.
