About this file

The document is a Contractor Work Surveillance Checklist for the placement or removal of an empty Rolloff Box (ROB) or Vacuum Box. The checklist is designed for a government representative to systematically evaluate and document a contractor's performance during container delivery, with specific criteria including:

The checklist requires verification of the contractor's punctuality, equipment readiness, container condition, weight documentation, and overall work quality. Key inspection points include confirming the container's appropriate size, presence of a plastic liner and watertight tarp (for ROB), container appearance and integrity, absence of significant damage, and safe equipment operation. The form provides spaces to note the contract number, task order, work dates, start and stop times, and allows the government representative to document comments and provide a signature, serving as an official record of the container delivery and inspection process.
