About this file

The document is a Contractor Work Surveillance Checklist for Industrial Cleaning Services (ICS) related to Contract Line Items (CLINs) 1002SV-1004SV and 1007SV-1009SV. The checklist is designed to comprehensively document and verify contractor performance during industrial cleaning operations, with specific focus on equipment, preparedness, and work quality. Key verification points include:

The checklist requires verification of multiple critical elements such as crew arrival time, equipment readiness, and operational effectiveness. Specific equipment requirements include a 3,500-gallon vacuum truck with 300 feet of hoses, a self-contained heated power washing unit, a liquid transfer pump, and appropriate personal protective equipment (PPE). Additional requirements include providing a weight ticket for the vacuum truck, government inspection prior to use, ensuring chemical compatibility of truck components with waste materials, and meeting Resource Conservation and Recovery Act (RCRA) empty standards. The form allows a designated government representative to systematically assess and document the contractor's compliance with technical and operational specifications during the industrial cleaning service engagement.
