PERFORMANCE WORK STATEMENT (PWS)

DEMOLITION SERVICES

1. DESCRIPTION OF SERVICES: There is a continuing need to demolish selected facilities at Altus AFB, Oklahoma. These are buildings and other facilities that are underused, deteriorated, or have outlived their expected service life, and which are often not economically feasible to renovate. There is also a cost to maintain such facilities. The purpose of this contract is to establish an expedient method for obtaining demolition services through a pre-selected contractor. Delivery method of such work is intended to be by task orders placed against an established general contract with a single selected contractor to provide the actual demolition. All work for this contract shall be performed at Altus AFB, Oklahoma.

1.1 SCOPE:

a. Basic scope of the specific work will be demolition of building B85, pavements, or other facilities associated with the said building.

b. Scope will include hauling away and disposal of debris generated by the demolition.

c. After demolition and hauling is complete, the Contractor will then grade the area and plant and establish ground cover using seed, sprigging, or sodding.

d. Overall intent of the demolition will be to remove the subject structure and establish open land with grass or other ground cover growing on it at the end of the work for each task order.

1.2 BASIC SERVICES:

a. Method of removal of structure shall be the prerogative of the contractor, if the removal method does not violate local, state, or federal laws, rules, or regulations regarding construction/demolition practices and disposal of demolition debris and materials.

Demolition shall be accomplished by mechanical means, using wrecking ball, bulldozer, front-end loader, backhoe, or other such machines to break up the structure and foundation for loading and hauling away. Explosive blasting shall not be allowed as any part of demolition task orders, nor shall any method requiring use of materials considered as or defined by the 97th Civil Engineer Squadron Environmental Flight as being hazardous.

b. Piers or foundations shall be cut off at least 6 feet below surrounding ground elevation, and clean fill dirt used to backfill and cover locations where holes are left in the ground.

Grade beams and other foundation parts less than six feet underground shall be removed completely.

c. Sidewalks in demolition may be designated to be demolished as part of the work, or saved or re-established after the demolition is completed, depending on the needs of the Government in each task order.

d. Parking areas connected with the facility may be designated to either be demolished or saved as part of each task order, depending on the needs of the Government.

Determination of these actions will be addressed in each task order.

e. Existing lawn sprinkler systems shall be preserved during the demolition if such are currently in place where the demolition is to occur. These will be made known to the contractor by the Government prior to the start of work, so the contract can take this factor into account in cost proposals.

f. The contractor shall take great care in avoiding damage to adjoining structures or pavements such as troop walks, sidewalks, roads, parking lots, curbing, lawn sprinklers, light poles, or any other ancillary structures in the area.

g. Demolition must include removing buried utility piping, drainage piping, sewer piping, electrical conduits, and any other utility ducting, etc. from the soil below the building up to a designated point outside of the building footprint. Utility lines shall be capped at the main before the service valve. In the absence of a service valve it shall be capped at the tee or wye for the facility. Electrical connections shall be removed back to the nearest switchgear.

h. Vegetation in the work area: Trees shall be preserved unless coordinated and approved by the contracting officer. Shrubbery and flower beds shall be removed with the demolition.

i. Debris shall be hauled away to a certified landfill site, and weight tickets for the deposited debris shall be rendered to the government for tracking purposes. Of special interest to the Government will always be hazardous materials encountered, removed, and disposed of during demolition activities. For more detailed information, see paragraph 4.6 Environmental.

j. All materials demolished, disassembled, or excavated during the execution of this project shall become the property of the contractor. The contractor may, and is indeed encouraged to, salvage any parts of the building (or other facility) that may be worthwhile to recycle or sell for scrap value. Examples would be sale of steel construction members for scrap, or recycling of electrical wiring, restroom items, or other components.

k. Establish grass after demolition is complete through placement of seed, sod, or sprigs in demolition area. New seed, sod, or sprigs shall be “buffalo grass” or other approved ground cover. The contractor shall be responsible for 14 days of watering and maintaining seeded site to ensure establishment of grass.

1.3 GENERAL REQUIREMENTS:

a. All work performed on this project shall meet local, state, federal and DOD codes.

b. The government depends on the existing as-built drawings provided by previous contractors for accuracy. The contractor is advised to field-verify all required information before providing a proposal.

c. The contractor shall be required to accomplish various small items of incidental work which may become necessary to complete the project in a satisfactory and workmanlike manner meeting the approval of the contracting officer or their designated representative.

The contractor shall include all such items of work as part of their detailed estimate for the work to be performed.

d. The contractor shall provide all labor, materials, travel, equipment, and supervision necessary to perform all work required for this project.

e. Damage to other buildings, utilities, or any other element during the execution of this project shall be repaired and restored to its original condition by contractor provided workers, skilled in trades involved, at no additional cost to government.

2. SERVICE DELIVERY SUMMARY: The contract service requirements are summarized

(see Table 1 – Service Delivery Summary) in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SDS and the Contractor’s Quality Control Plan provides information on contract requirements, the expected level of Contractor performance and the expected method of Government surveillance and confirmation of services provided. These thresholds are critical to mission success.

Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1 QUALITY ASSURANCE: The Contracting Officer Representative (COR) shall inspect and evaluate the Contractor’s performance to ensure services are IAW requirements set forth in the contract. The COR shall use surveillance/performance assessment methods as dictated by the Performance Plan (PP). The COR will use the Contractor’s work schedule or modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the Contractor’s performance. When the Contractor performance has not been acceptable, the COR will notify the Contractor’s Quality Control. The service shall be re-performed by the Contractor and re-inspected by the COR. When a Performance Threshold has not been met the COR will annotate the surveillance accordingly.

2.2 PROGRESS MEETINGS: The Contracting Officer, Project Manager, COR, or other Government personnel (as appropriate) and the Contractor shall meet periodically to discuss the Contractor’s performance/progress. The following issues shall be discussed: opportunities to improve the contract, any modifications required of the contract, and progress of the various projects. The minutes of these meetings will be reduced to writing, signed by the CO and any other signatures as deemed appropriate, distributed to the Project Manager, the COR, and the Contractor. Should the Contractor not concur with the minutes, the Contractor will provide a written notification to the CO identifying areas of non-concurrence for resolution.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES:

3.1 GOVERNMENT FURNISHED PROPERTY: N/A

3.2 GOVERNMENT FURNISHED SERVICES: The Government will provide police and fire protection on Altus AFB. In the event of a medical emergency, call “911” for transportation of an injured employee to the local hospital. The Government will provide emergency medical treatment on site for life-threatening emergencies only until a civilian ambulance arrives.

4. GENERAL INFORMATION:

4.1 REQUIREMENTS:

a. Known government records of tests for asbestos containing materials (ACM) and/or lead-based paint (LBP) will be provided to the contractor as part of each task order bid package, as applicable. The contractor may choose to retest any area suspected of or reported as having ACM or LBP to confirm that these areas are correctly designated. It shall be the contractor’s responsibility to deal with known ACM and LBP (or other known hazardous materials, such as cooling refrigerants or electrical cooling fluids; e.g.

PCB’s) in accordance with local, state, and federal laws, rules, and regulations.

Hazardous material demolition and disposal shall be accomplished only by certified hazardous material handling personnel. If contractor personnel are so certified, they may perform this work under over watch of 97th CES Environmental Flight certified personnel.

b. Utility shutoffs for the demolition shall be coordinated with 97th Civil Engineer Squadron and 97th Communications Squadron for applicable utilities.

c. For mechanical systems, anywhere there is Freon or other such coolant(s) in the system, they must be removed and disposed of in accordance with Local, State, and Federal rules, regulations, and laws.

d. For electrical demolition, if there is a liquid-cooled transformer or other such equipment to be removed as part of the demolition, the coolant must be identified and removed/disposed of in accordance with Local, State, and Federal rules, regulations, and laws.

4.2 TESTS TO BE PERFORMED:

a. Asbestos or Lead-Based Paint testing, if necessary.

b. Compaction tests for compacting any fill or graded material.

4.3 SUBMITTALS:

a. The contractor shall provide the following information to the contracting officer or their representative at the end of the project:

i. Provide as-built “red-line” drawings showing (at a minimum) locations where utility lines, drains, and service lines are capped.

ii. Test results for any hazardous material testing.

b. If the work on the task order is to exceed 60 calendar days, a demolition schedule (work plan) shall be submitted for approval to the Contracting Officer at least 7 days prior to the projected start of actual work.

4.4 PRE-WORK REQUIREMENTS AND DUTY HOURS:

a. The contractor shall notify the contracting officer minimum 10 working days prior to beginning any demolition or excavation in order to obtain AF Form 103, Work Clearance Request, also referred to as a “digging permit”.

b. Normal working hours for contractors is 7:30 AM to 4:30 PM Monday through Friday, excluding federal legal holidays.

i. FEDERAL HOLIDAYS: The Contractor is not required to provide service on federal holidays, except in emergency situations, as determined by the CO. The holidays are:

• New Year’s Day - 1 January

• Martin Luther King Day - 3rd Monday in January

• President’s Day - 3rd Monday in February

• Memorial Day - last Monday in May

• Independence Day - 4 July

• Labor Day - 1st Monday in September

• Columbus Day - 2nd Monday in October

• Veteran’s Day - 11 November

• Thanksgiving Day - 4th Thursday in November

• Christmas Day - 25 December

NOTE: If a holiday falls on a Saturday, the preceding Friday will be observed. If a holiday falls on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day after the holiday observance.

c. The contractor shall notify the contracting officer a minimum of 5 working days prior to scheduling any non-duty work hours to obtain permission to do so.

4.5 CONTRACTOR MANPOWER REPORTING: The Contractor shall report all Contractor labor hours (including sub-Contractor labor hours) required for performance of services provided under this contract for “U.S. Air Force” via a secure data collection site. The Contractor is required to completely fill in all required data fields using the following web address http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September

30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the electronic Contractor Manpower Reporting Application (eCMRA) help desk.

4.6 ENVIRONMENTAL:

a. SPILLS – Any spills of petroleum, oil, and lubricant (POL) products, chemicals or other hazardous materials shall be reported immediately to the appropriate authority as follows:

i. If the maximum spill dimension in any plane is up to 24”, contractor shall clean up the spill immediately and inform Civil Engineer Asset Management Flight by contacting Mrs. Lori Stevens (580) 481-7605 or Mr. Dave Wallace at (580) 481- 5066.

ii. If the maximum spill dimension in any plane is greater than 24” or the spill is of continuous nature, contractor shall call 911 immediately and provide the following information - nature of emergency, exact location of the incidence, call back number and any other information required by the 911 dispatcher. The dispatcher will route the call to Altus AFB, OK Fire Department.

b. DEBRIS – The contractor shall remove all project debris from Altus AFB, OK and submit weight or load tickets for construction debris to the project inspector.

i. The contractor shall maintain a clean and safe work site and remove excess debris at the end of each workday.

ii. All materials demolished, disassembled, or excavated during the execution of this project shall become the property of the contractor. The contractor shall refer to "Environmental Requirements for Construction at Altus Air Force Base, Section 007200, Rev 01 July 2011 or later" as the guide for requirements concerning recycling, reuse, and disposal of demolished or dismantled materials and equipment, and reporting on disposition of these items. Adherence to the requirements of this specification and all of its sections and subsections shall be http://www.ecmra.mil/ considered as part of this project. The contractor may refer any and all questions pertaining to this specification to the Environmental Flight of 97th CES by contacting Mr. Mike Reyes at (580) 481-6221.

c. LOCAL INSTALLATION ENVIRONMENTAL MANAGEMENT SYSTEM/POLICY – Altus AFB has implemented an Environmental Management System (EMS) under the guidelines of ISO14001and AFI 32-7001, Environmental Management. Every contract employee that will physically work on the installation is required to review and understand the Altus AFB Contractor EMS Awareness PowerPoint presentation. The training slides and general environmental guide are accessible on the Altus AFB off-base website at www.altus.af.mil. These slides and guide provide a general overview of the wings EMS plus provide specifics on some potential environmental impacts associated with generic contract work. Any new or replacement employee shall receive this training prior to commencing work on Altus AFB. Proof of completion shall be forwarded to the COR no later than five (5) business days from the day the employee begins work. Questions regarding the training and alternative means to complete the training should be addressed to 97 CES/CEIEC, (580) 481-6221.

d. ASBESTOS – Asbestos-containing material (ACM) may be present in the work area.

This may consist of existing roof insulation, part of existing built-up roofing materials, part of the existing bitumen, or other materials in the project work area. Attached to the Environmental Certification, provided in copy to the Contractor, is the most recent evaluation of possible ACM in the work area. If the contractor locates any material, he suspects of being ACM, he shall have such material tested for ACM by a certified asbestos testing lab to confirm or deny the presence of asbestos. If additional asbestos is found to be present, then the contractor shall engage the services of a certified asbestos removal contractor to remediate the material that is found. Compensation for this additional work will be negotiated between the government and the contractor.

4.7 LOCAL ANTI-TERRORISM AWARENESS INFORMATION: Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. To help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

4.8 INFORMATION PROTECTING SECURITY INFORMATION:

a. Pass and Identification Items - The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:

i. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not be limited to, the following: full name to include first, middle, and last, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus

AFB.

ii. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.

iii. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract.

Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.
