About this file
This document is a Past Performance Questionnaire template for RFP 80LARC26R0002, issued by NASA Langley Research Center for the Geostationary Littoral Imaging Monitoring Radiometer (GLIMR) Access to Space (ATS) contract. The questionnaire is designed to evaluate contractor past performance and is classified as source selection information per FAR 3.104. Offerors must complete Section I with basic contract information about their proposed past performance references, including company name, contract number, total contract value, period of performance, contract type, and any award/incentive fee ratings received over the past three years. Sections II through IV are completed by government and/or industry customer evaluators who assess the contractor's performance using a defined rating scale.
Evaluators rate contractor performance across eight specific elements relevant to space access and satellite operations: orbital parameter maintenance, contamination control, integration and pre-launch processing, ground system design and testing, spacecraft bus and launch vehicle provision, checkout and operations, ground system data delivery, and design review support. Ratings use a six-point scale from Exceptional (E) to Unsatisfactory (U), with Not Applicable (NA) as an option. Evaluators also rate the amount of work performed in each area using a four-point scale from Significant to Minimal. Additionally, four general performance categories are assessed: quality of product/service, schedule, business relations, and utilization of small business, culminating in an overall performance rating. The questionnaire requests that responses be completed jointly by technical customers and contracting officers when possible, with completed forms submitted directly to Ashley Korahaes at larc-glimr-ats@mail.nasa.gov. References should include existing Contractor Performance Assessment Reports (CPARS) from the past five years as of July 22, 2026.
