About this file
This is an Experience Modification Rate Form that offerors must complete and return with their response to a federal solicitation.
The form requires offerors to provide company identification information including name, address, telephone, email, and primary contact. Offerors must complete a three-year data table (2023-2025) using OSHA 300 Forms to report: total man hours worked at jobsite and office locations; number of cases involving days away from work, restricted activity, or both (Columns H and I of OSHA 300); the Days Away, Restricted, or Transferred (DART) Rate calculated using the formula of cases multiplied by 200,000 divided by total man hours; and the number of serious, willful, or repeat OSHA violations within the preceding three years with explanatory documentation required for any violations. Offerors must also submit copies of their OSHA 300 and 300a Forms. Additionally, offerors must provide their six-digit North American Industrial Classification System (NAICS) Code, identify the individual who administers the company's Safety and Health Program, and disclose their Insurance Experience Modification Rate (EMR) for the past three years. The form specifies that an EMR greater than 1.0 may result in disqualification. The government will cross-reference submitted information with OSHA and EPA online inspection history databases to verify the contractor meets the Experience Modification Rate requirements stated in the solicitation.
