STATEMENT OF WORK

Replace Doors 3 -

Project Number 653-21-104

1.0 DESCRIPTION OF WORK

BACKGROUND: This project includes general construction, alterations, demolition, exterior and interior improvements necessary for the Replace Doors 3, and other items necessary to complete the project.

Contractor will completely prepare the site for construction operations, including demolition and removal of existing structures, furnish labor and materials, perform work for the Installation and

Repair of Electronic doors.

The project is located on the VA Roseburg Healthcare System main campus at 913 NW Garden

Valley Blvd, Roseburg, OR 97471. The site and campus buildings are listed as eligible for the

National Historic Register.

All work on the contract shall be performed between 8:00 a.m. and 4:30 p.m., Monday through

Friday, excluding National Holidays, unless approved by the Contracting Officer (CO).

Contractors may request, in writing, approval to work other hours or weekends or may be directed to work other hours. Except for emergencies, the COR should receive such requests two weeks before the scheduled work. When possible, Contractors will submit emergency requests at least two business days before the scheduled work.

1.0.1 General - Project includes the following:

• Install new doors per plan, verify and coordinate prior to ordering.

• Install wall mounted or remote door actuators as needed.

• Provide power to the wall or remote mounted actuators and Automatic Door Opener

(ADO) system as needed

• Repair ceiling, floor systems and base molding to match existing as needed.

• Perform all work in accordance with specs and drawings.

1.0.2 Construction Period of Performance: Contractor will be provided a 30 day limited notice to proceed after pre-construction kick-off meeting for materials submittals and ordering period. The Notice to Proceed will be issued after all materials have been delivered for build out. Build out time is expected to last 90 consecutive days.

1.0.3 Project Management: This project will require a high level of coordination with VA staff and regularly scheduled project meetings will be required to facilitate communications for this project.

1.0.4 Construction Inspection: The Contractor will develop and implement a Quality

Assurance/Quality Control Plan that includes project inspection for the duration of the construction period of the project in accordance with specifications.

1.0.5 Construction: The Contractor will provide all labor, materials and equipment for demolition and installation in construction project areas.

1.1 Life Safety: The Contractor will design interim life safety measures as part of the construction documents.

1.2 Contractor shall develop a “Submittal Register” based on the included specification sections. After submittals are reviewed for compliance, the Contractor shall log them in the submittal register. A copy of the submittal register will be returned with the submittals.

1.3 Submittals shall be submitted 30 working days, excluding federal holidays, prior to proceeding with that portion of the contract work which requires submittal approval.

Delays attributable to untimely and rejected submittals will not serve as a basis for extending contract time for completion. Partial submittals will not be accepted unless authorized by the Contracting Officer or COR.

1.4 Project Management

1.4.1 Project meetings will be held on a weekly basis or more frequently as needed.

1.4.2 The contractor will plan, coordinate the project meetings, and take notes of the meeting to distribute to all stakeholders after the meeting.

1.4.3 VA meeting room facilities may be utilized for project meetings. The Contractor will coordinate with the COR for any use of VA facilities.

1.4.4 The Contractor will have an authorized representative on the project site at all times during construction. The authorized representative will have the authority to receive instruction from the Government and direct the actions of Contractor employees and all subcontractors.

1.4.5 The COR shall monitor technical aspects as outlined in the COR Delegation issued by the CO The COR shall not change any terms and conditions of the contract. Any changes to work or additional work shall be made in writing by a Network

Contracting Office 20 (NCO 20) CO.

1.4.6 The COR for the project will be identified at the pre-construction kick-off meeting.
