About this file
This is a Quality Assurance Surveillance Plan (QASP) Version 1.0 for an Aircraft Computerized Maintenance Manager (CMMS) Operations, Maintenance, and Enhancement (OME) contract. The QASP is a government-developed document designed to ensure the contractor meets performance-based acquisition standards outlined in the associated Performance Work Statement (PWS) through systematic quality assurance methods and oversight.
The USDA will implement surveillance activities through designated personnel: Contracting Officer Cori Gonzales (corrine.gonzales@usda.gov) and Contracting Officer's Representative Stephanie Stone (stephanie.stone@usda.gov, 970-502-8996), who is responsible for technical administration and contractor performance monitoring. The COR may designate additional inspectors to assist with oversight. The contractor bears responsibility for developing its own Quality Control Plan to meet PWS standards, while the QASP provides independent government surveillance. The COR will document all monitoring activities using the Performance Monitoring Results Template on a quarterly basis and whenever performance fails to meet acceptable quality levels. Four performance requirements are specified with measurable Acceptable Quality Levels: PR 1.1 requires CMMS full functionality 95% of the time; PR 2.1 requires 24/7/365 support via telephone and/or email at 100% compliance; PR 2.2 requires one user training session at 100% completion; and PR 2.3 requires 100% responsiveness to integration and configuration changes. The QASP establishes five performance rating categories (Excellent/Outstanding, Very Good, Satisfactory, Marginal, and Unsatisfactory) to evaluate contractor performance, and all deviations to targeted performance requirements require pre-approval by the Contracting Officer.

Quality Assurance Surveillance Plan (QASP) V1.0
Aircraft Computerized Maintenance Manager(CMMS) Operations, Maintenance, and Enhancement (OME)
1. Purpose
This Quality Assurance Surveillance Plan (QASP) is a Government developed document used to ensure that systematic quality assurance methods are used in the administration of the Performance-based Acquisition (PBA) standards included in this Performance Work Statement (PWS). The intent is to ensure that the Contractor performs in accordance with the targeted performance requirements contained in the PWS; that the Government receives the quality of services called for in the contract; and, that the Government only pays for the acceptable level of services received.
2. Scope
It is important to understand the responsibilities of the parties and distinguish between the Quality Control Plan (QCP) and the QASP. The contractor is responsible for managing and developing a quality control plan to meet the quality standards established in the PWS. The QASP is put in place to provide Government surveillance and oversight of the contractor’s efforts in meeting the performance standards required by the contract. The QASP is not part of the contract nor is it intended to duplicate the Contractor’s quality control plan. The Government may provide the Contractor with an informational copy of the QASP to support the Contractor’s efforts in developing a quality control plan that will interrelate with the QASP.
3. Government Resources
The following personnel will implement all surveillance activities:
Contracting Officer. The Contracting Officer ensures performance of all necessary actions for effective contracting, ensures compliance with the terms of the contract, and safeguards the interests of the United States in the contractual relationship. The Contracting Officer assures the Contractor receives impartial, fair, and equitable treatment under the contract.
The Contracting Officer is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.
Contracting Officer:
Email:
Cori Gonzales corrine.gonzales@usda.gov
Contracting Officer’s Representative (COR). The COR is responsible for technical administration of the contract and assures proper Government surveillance of the Contractor’s performance.
mailto:mark.mohler@usda.gov
The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the Contractor deems may affect contract price, terms, or conditions shall be referred to the Contracting Officer for action.
Contracting Officer’s Representative: Stephanie Stone Email Address: stephanie.stone@usda.gov Telephone Number: (970) 502-8996
COR Designees. Additionally, COR Designees, also known as Inspectors, may be named to ensure that the Government sufficiently directs, oversees, measures and monitors contractors.
COR Designees will provide information to the overall contract COR.
4. Methods of Quality Assurance Surveillance
A combination of the surveillance methods shall be used in the administration of this QASP. The specific surveillance method and the targeted performance level for each performance requirement is listed in the PWS, Section 18: Performance Requirements Section.
COR /COR Designee Review – Indicates that the COR and COR Designee will be responsible for monitoring the Contractor’s performance in meeting specific performance standards/acceptable quality levels.
Customer Feedback – Customer feedback may be obtained either from the results of formal customer satisfaction surveys or from customer complaints received by the Forest Service.
Customer complaints, to be considered valid, must set forth clearly and in writing the detailed nature of the complaint, and must be signed and forwarded to the COR, who shall assess the validity of the complaint and retain it in a documentation file. The COR shall ensure the tabulated results of all customer satisfaction surveys are kept on file.
5. Performance Monitoring Results
The results of all monitoring activity shall be documented on the Performance Monitoring Results Template (Attachment 09). The COR shall submit copies of the completed Performance Monitoring Results to the Contracting Officer on a quarterly basis, at the conclusion of contract performance, and whenever contractor performance fails to meet acceptable quality levels.
6. Documentation
The COR will, in addition to providing documentation to the Contracting Officer, maintain a complete quality assurance file. The file will contain copies of all reports, evaluations, recommendations, surveillance checklists, and any actions related to the Government’s performance of the quality assurance surveillance function. All such records will be retained for the life of the contract and forwarded to the Contracting Officer upon completion.
mailto:stephanie.stone@usda.gov
7. Deviations to Target Performance Requirement
Deviations to the targeted performance requirements require preapproval by the CO. All deviations shall be documented in the Performance Monitoring Results.
8. Surveillance Matrix
PWS section 4: Performance Requirements Section contains the list of performance objectives and standards that must be performed by the contractor. This matrix details the method of surveillance the COR/COR Designee will use to validate and inspect these performance elements. In addition, the government or a third-party vendor may perform code inspections.
Inspection of each element will be documented in the COR file.
Performance objectives define the desired outcomes. Performance Standards define the level of service required under the contract to successfully meet the performance objective. The inspection methodology defines how, when, and what will be assessed in measuring performance. The Government performs surveillance, using this QASP, to determine the quality of the contractor's performance as it relates to the performance element standards. PWS, Section E: Performance Requirements section should be used to form the foundation of the COR's inspection checklist.
In evaluating the quality of contractor’s performance, the following performance ratings may be used.
Performance Rating Criteria
Excellent / Outstanding
Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
Very Good Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor was effective.
Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
