Attachment 3 - Contactor Requirements Submission Matrix
PWS Ref.PWS RequirementINSTRUCTIONS: Quoters shall provide the certifications or other information as part of its Technical Volume to demonstrate how its proposed approach meets the below requirements. In each line below, provide a brief statement of how the proposed approach meets the requirements and identify where the supporting information is contained in your Technical Volume.
Certifications/Approvals/Accreditations. The contractor shall have the following certifications/approvals/accreditations:
PERSONNEL:
3.2.1Certified Precision Oxygen Cleaning worker (onsite), a minimum of one (1) employee.
3.2.2Certified technician for cleaning, operation, maintenance and repair of various oxygen booster pumps (IE: Haskell, Hydraulics International), a minimum of one (1) employee.
3.2.3D.O.T. designated hydrostatic test operator, a minimum of one (1) employee.
3.2.4Certified technician for retail service, maintenance and repair of divers breathing air compressors (IE: Bauer, Rix). A minimum of one (1) employee
FACILITIES:
3.2.5Hazmat and air quality control board requirements compliances in accordance with Federal, State and Local jurisdiction in which they are located.
3.2.6Oxygen Instrument Calibration Room. Approved by competent authority (IE: NAVSEA)
3.2.7Oxygen Cleaning Facility (Class B) IAW MIL-STD-1330D CH1. Approved by competent authority (IE: NAVSEA)
3.2.8Oxygen Cleaning Process IAW MIL-STD-1330D CH 1 for component, cylinder and piping/hose cleaning. Approved by competent authority (IE: NAVSEA)
3.2.9Calibration services accreditation IAW OPNAVINST 3960.16
3.2.10ISO/IEC 17025:2005 or ANSI Z540.3 or ISO 9001:2015 or in-place Quality Management System that meets the intent of the standard
3.2.11D.O.T. approved cylinder requalification facility with Grade B water for conducting hydrostatic testing of oxygen cylinders
3.2.12Certifications and permits required to handle Freon for gauge cleaning affluent sampling as applicable at Federal, State and Local level
3.2.13Space free from exposure to dust, debris and other environmental conditions which would adversely impact the cleanliness of DLSS.
