About this file

This file is a template for the Monthly Active Roster Report required under a Blanket Purchase Agreement (BPA) for gym membership services in Australia. The template is designed for vendors to submit monthly reports to the Contracting Officer and Authorized Callers documenting all active U.S. Government gym memberships during the preceding month, with submission required by the 10th calendar day of each month once at least one active membership exists under the contract.

The roster template requires vendors to report membership details including membership ID, assigned personnel name, membership structure (Preferred Transferable Slot or Standard Individual), duration type (Month-to-Month, 3-Month, or 12-Month), access level, primary facility location, term start and expiration dates, and current status. The template includes a monthly summary section for tracking total counts of each membership type and accounts on hold. Membership structure designation is significant, as "Preferred Transferable Slot" represents the Government's preferred model allowing flexibility in personnel assignment, while "Standard Individual" memberships are non-transferable secondary options. The template provides example entries showing the expected format and includes instructional notes directing vendors to align duration types with the Performance Work Statement and price list parameters, emphasizing that reporting is only required once the contractor has activated at least one membership account under the BPA.
