About this file
This is a Government Format Pricing Model (GFPM) for Prime Contractors used to submit detailed cost proposals for the Medical Instructor/Operator Support Services (MIOSS) contract (W900KK26RA017). The GFPM is a comprehensive Excel-based template requiring offerors to input labor rates, hours, indirect costs, and other cost elements across a base period and four one-year option periods, with calculations for a six-month extension option under FAR 52.217-8. The contract requires staffing Instructor/EMT and Instructor/Paramedic positions across 23 military installations (both CONUS and OCONUS locations) with a total of 1,113,860 required labor hours spanning approximately 4.5 years. Direct labor floor rates are provided by location and specialty, ranging from $26.59 to $50.34 per hour, and offerors must propose fully burdened labor rates incorporating direct labor, fringe benefits, labor overhead, general and administrative costs, and profit. Offerors must also account for DBA insurance costs (if applicable), propose indirect rates for labor and other cost categories (though material and ODCs are not applicable to this acquisition), and include any program management costs developed in their own pricing models.
The GFPM accommodates both prime contractor and subcontractor labor allocations, with subcontractors required to submit a separate GFPM-Subs document detailing their full cost buildups and DBA insurance calculations. Cost proposals are organized by Contract Line Items (CLINs) and include mandatory staffing levels by position and location, with surge capability calculations available if specified in the solicitation. The template auto-calculates total proposed prices, ceiling rates (set at 5% above proposed FBLR for CONUS positions and 10% for OCONUS), and generates summary tables organizing costs by period of performance. The model is structured to support Government evaluation of cost reasonableness and realism, with instructions emphasizing that reconciliation differences between prime and subcontractor submissions will default to prime contractor data for evaluation purposes.

INSTRUCTIONS TO OFFERORS
| General | |
| 1 | The Prime Offeror shall complete this Government Format Pricing Model (GFPM). |
| 2 | For proposal purposes, Offerors shall assume that a productive hour work year is equal to the hours per LCAT shown in column G of each performance period. |
| 3 | If offerors choose to provide additional cost data as a separate submission, offerors shall provide reconciliation between the contractor format spreadsheets and the Government format spreadsheets. If reconciliation error(s) or differences exist, the Government will rely on the data entered into the Government format spreadsheets to make any reasonableness/realism determinations. |
| "Totals" Tab | |
| 1 | Offerors shall input offeror name into the applicable yellow-highlighted cell in row 1 |
| 2 | RESERVED (no offeror instruction currently) |
| 3 | The FAR 52.217-8 6-Month "Option to Extend Services" is automatically calculated by multiplying the last option period prices by 50.00%. |
| 4 | The total proposed price is calculated within this tab, at the end of the populated price data, in column G next to the "Total Proposed Price" title written in column C. |
| 5 | If applicable, the "Totals" tab also shows the "Total Proposed Price less DBA Insurance " title, at the end of the populated price data, written in column C, showing a mathematical calculation in column G that excludes DBA Insurance from the Total Proposed Price. This is included in case the Government decides to exclude DBA Insurance Costs from the Total Evaluated Price (will be defined in the solicitation if applicable). |
| 6 | No other offeror input necessary. All totals will auto-populate from the other tabs, as applicable. |
| "Base Period - Labor", "Option Period 1 - Labor", "Option Period 2 - Labor", "Option Period 3 - Labor", and "Option Period 4 - Labor" Tabs | |
| 1 | Offeror input is required in all applicable cells highlighted in yellow. |
| 2 | The "Labor Categories & Required Hours" (columns A - I) contain all of the performance locations and labor categories that the USG anticipates to be necessary for completion of this effort. The "Req. Hours ( )" column G contains the total number of hours to be proposed for each of the USG identified labor categories. The hours may be allocated to the prime contractor or a combination of both the prime and subcontractors. All proposed (prime & subcontractor) labor hours must be equal to the total number of hours (per each labor category) identified in the "Req. Hours ( )" column. |
| 2a | For each year, offerors will input as necessary on the CLIN TAB the total cost & profit associated with Program Management costs developed and detailed in its OFPM. |
| 2b | In the "Prime Contractor Labor" section (columns K-X), offerors shall input the proposed number of hours that will be completed by the prime contractor for each of the USG identified labor categories. Offerors are required to submit fully burdened labor rates for each of the USG identified labor categories (Column U). Offerors shall insert all of the cost elements (direct labor, fringe benefit costs, labor overhead costs, general & administrative cost, profit, etc.) that make up all of the fully burdened labor rates proposed for each of the USG identified labor categories (Columns M-T in the Base Period tab and Columns O-T in Option tabs). Column N is pre-populated in the Option Period labor tabs based on the proposed direct labor rates in the Base Period labor tab in conjunction with the "Escalation" tab. Columns S and U-X automatically calculate based on the offeror's inputs. |
| 2c | Offerors shall insert direct labor rates in in column M within the Base Period (BP) - Labor tab for all required labor categories. |
| 3a | In the "Subcontractor Labor" table (columns Z-AL), offerors shall input the proposed number of hours that will be completed by the subcontractor for each of the USG identified labor categories. For labor categories that Offerors are proposing subcontractor hours for, Offerors are required to submit fully burdened labor rates (in column AA) for each applicable labor category. If applicable, Offerors shall insert any other cost elements associated with "prime adders" in columns AB-AD and AF. Columns AE and AG-AJ automatically calculate based on the offeror's inputs. |
| 3b | In column AK, the prime offeror shall enter the name of the subcontractor(s) proposed to complete the hours associated with each labor category. If the prime offeror proposes more than one subcontractor for a single labor category a single FBLR shall be proposed. The proposed FBLR shall be a blend of each subcontractor’s proposed FBLR. In column AL the prime offeror shall indicate if the proposed subcontractor FBLR is a "blended" FBLR. |
| 4 | Columns AM-AO calculate a single composite fully burdened labor rate (prime and subcontractor) for each of the USG identified labor categories. |
| 5 | Only if applicable, the USG has included surge capability by multiplying the total labor price by the surge percentage shown at the end of the populated price data, in column C next to the "Surge Percentage" title written in column B. |
| "BP - Material Travel ODC", "OP 1 - Material Travel ODC", "OP 2 - Material Travel ODC", "OP 3 - Material Travel ODC", and "OP 4 - Material Travel ODC" Tabs | |
| 1 | The Government pre-selects its evaluation strategy related to Material, Travel, and/or ODCs. Material, Travel, ODCs can be "plug" figures, determined by the Government or "proposed" by an Offeror. The Government has also developed a "discount" strategy related to M&IE and Per Diem in cases the Government determines there may be cost savings opportunities related to M&IE and Per Diem. The applicable Material, Travel, ODC strategy will be defined in the solicitation and the GFPM will be tailored accordingly. Material and ODCs are NOT APPLICABLE to this acquisition. Travel and Surge are PLUG numbers and shall not be changed by the Offeror. |
| 2 | Only as applicable, The Material/ODC/Travel Plug Figure(s) highlighted in green are pre-populated plug figures. IF the Government is requiring an Offeror to propose its Material, ODC, and/or Travel costs, the applicable cells will be highlighted yellow for offeror input(s). |

| 7 | Only if applicable, Offerors shall input its estimated DBA Insurance Amount in in column E next to the "DBA Insurance (Cost Only)" title written in column A. Note that the DBA Insurance Amount should include DBA costs for both the Prime and any applicable subcontractor. |
| 7a | Only if applicable, The USG has included surge capability by multiplying the total DBA cost by the surge percentage in column B next to the "DBA Insurance - Surge" title written in column A. |
| 8 | If applicable, Offerors may propose a DBA Management Fee in column E next to the "Management Fee (FFP)" title written in column A. If a contractor chooses to propose a FFP, it is intended to offset the instruction that indirect costs are not allowable to be applied to DBA costs in performance. |
| "Escalation" Tab | |
| 1 | This tab provides labor escalation rates which apply to all option periods of performance tabs. |
| 2 | The Government has set a minimum escalation for each period of performance. Offerors may choose to increase this rate in column C of the "Escalation" tab, but shall not propose an escalation rate which is below the USG minimums. |
| "Composite Labor Rates" Tab | |
| 1 | This tab organizes all proposed composite (prime and subcontractor) costs, profit and fully burdened labor rates. All values automatically populate based on offerors input on the "Base Period" and all "Option Period" tabs. |
| 2 | No offeror input is necessary. |
| "Prime Ceiling Rates" Tab: | |
| 1 | If applicable, the Government is requiring prime contractors to propose a cost ceiling on their fully burdened labor rates (FBLRS) (less profit), where the ceiling is equal to the proposed FBLR (less profit) plus 5% for each CONUS labor category or plus 10% for each OCONUS labor category. This tab includes a formula that auto-calculates these ceiling rates for each period. |
| 2 | No offeror input is necessary. |
| "Subcontractor Ceiling Rates" Tab: | |
| 1 | If applicable, the Government is requiring prime contractors to propose a cost ceiling on their subcontractor fully burdened labor rates (FBLRS) (less PROFIT), where the ceiling is equal to the proposed subcontractor FBLR (less PROFIT) plus 5% for each CONUS labor category or plus 10% for each OCONUS labor category. This tab includes a formula that auto-calculates these ceiling rates for each period. In circumstances related to subcontractors who are on-boarded after performance begins (those not proposed during proposal submission), this tab shows the ceiling rates per labor category for any subcontractor who performs work under the proposed subcontractor allotted labor categories. |
| 2 | No offeror input is necessary. |
| "CLINs" Tab: | |
| 1 | This tab organizes all proposed costs and Profit in a format aligned with a suggested CLIN structure. Subject to change with award. |

| "eCRAFT" Tab: | |
| 1 | RESERVED (no offeror instruction currently) |
| Subcontractor Attachment - GFPM (Subs) | |
| 3a | The Government has included a separate subcontractor GFPM attachment (GFPM-Subs) for the prime offeror to provide each subcontractor. The subcontractor GFPM shall be utilized for each subcontractor to show its compliance with providing a full labor buildup, per labor category and to ensure compliance with the Government provided floor rates. |
| 3a | Subcontractor offerors shall submit a detailed cost build up for each proposed fully burdened labor rate utilizing the GFPM-Subs attachment. Subcontractors may choose to submit the cost build up data as part of the prime contractor's cost submission or as a separate submission directly to the Government. If reconciliation differences exist between the prime contractor's pricing submission and the subcontractor cost build up spreadsheets, the Government will rely on the data entered into the prime contractor's Government format spreadsheets to make any reasonableness/realism determinations. |
| 3b | All subcontractors must provide the subcontractor GFPM (GFPM-Subs) to the prime offeror for submittal to the Government as part of its prime offeror proposal OR each subcontractor must submit this file directly to the Government. In either case, it is the prime offeror's responsibility to ensure the Government receives this subcontractor GFPM. |
| 3c | To promote consistency between subcontractor files and prime offeror files, the Government has provided a "Sub - FBLR Summary" tab within the GFPM-Subs for the subcontractor to be able to more easily transfer its proposed FBLRs, for its provided labor categories, to the prime offeror for the prime offeror to include in its proposal. |

Subcontractor Instructions
INSTRUCTIONS TO SUBCONTRACTORS
| General | |
| 1 | The subcontractor shall complete this Government Format Pricing Model (GFPM-Subs). |
| 2 | For Proposal Purposes, Offerors shall assume that a Productive Hour Work Year is equal to 1,920 Hours CONUS and 3,744 Hours OCONUS. |
| 3 | All proposed direct labor rates shall be in line with justifying data (historical/incurred costs, salary survey, BLS OES etc.) for each identified locality. |
| 4 | The subcontractor GFPM will be utilized for each subcontractor to show its compliance with providing a full labor buildup, per labor category and to ensure compliance with the Government provided floor rates. |
| "Subcontractor Labor Build" Tab | |
| 1 | All subcontractors must provide the subcontractor GFPM to the prime offeror for submittal to the Government as part of the prime offeror proposal OR each subcontractor must submit this file directly to the Government. |
| 2 | Offeror input is required in all applicable cells highlighted in yellow. Option Period proposed direct labor rates (J62-J285) auto-calculate based on the proposed direct labor rate in the preceding period and the proposed escalation rate for the applicable Option Period and are locked. |
| 3 | The "Labor Categories & FTEs" table (columns A - F) contain all of the performance locations and labor categories that the USG anticipates to be necessary for completion of this effort. Offerors shall input the proposed number of Full Time Equivalents (FTEs) in column F that will be completed by the subcontractor for each applicable labor category being performed by the subcontractor. |
| 4 | In the "Subcontractor Labor Build" table (columns H-U), Column H auto-calculates based on the number of FTEs in column F. Subcontractors are required to submit fully burdened labor rates for each of the labor categories that the subcontractor is performing (Column R). Subcontractors shall insert all of the cost elements (direct labor, fringe benefit costs, labor overhead costs, general & administrative cost, profit/fee etc.) that make up all of the fully burdened labor rates proposed (Columns L-Q). If necessary, subcontractors may submit additional cost data in it's own format to further justify the proposed single elements of the prime labor costs. |
| 5 | Offerors shall insert direct labor rates for the Base Period for all labor categories (J5-J57). Note: Per section L, the proposed direct labor rates must meet or exceed the USG floor rates referenced in column I (I5 - I57). |
| 6 | In the event a subcontractor proposes a direct labor rate below the floor rate in error, the subcontractor GFPM corrects this error by utilizing the floor rate (as calculated by the pre-populated formula in column K) to compute the subcontractor costs that contribute to the subcontractor proposed FBLR in columns L-Q. |
| 7 | Profit / fee formulas in column Q restrict subcontractors from proposing negative profit/fee percentages. |
| 8 | Particular cells are locked and cannot be edited. Cells highlighted in yellow can be modified to fit a subcontractors accounting methods, however, the Government has included pre-populated formulas if applicable. |
| 9 | The FBLR proposed in column R should be consistent with the FBLR provided to the prime for the primes submittal of its proposal. |
| "Escalation" Tab | |
| 1 | This tab provides the data for the labor escalation rates which apply to all option periods of performance tabs. |
| 2 | Per section L, for evaluation purposes only, this solicitation has set a minimum escalation for each period of performance. Subcontractors may choose to increase this rate in column C of the escalation tab, but shall not propose an escalation rate which is below the USG minimum. |
| "DBA Insurance" Tab | |
| 1 | Subcontractors shall demonstrate on the DBA Tab of the GFPM-Subs how the estimated DBA Insurance Amount was calculated, if applicable. |
| 2 | DBA Insurance shall not be included in any FBLRs and may not have indirect expense or fee added to it. |
| 3 | Subcontractors shall provide a copy of the DBA Insurance quote, if applicable. |
| "Sub FBLR & DBA Summary" Tab | |
| 1 | To promote consistency between subcontractor files and prime offeror files, the Government has provided a FBLR & DBA Summary tab within the Subcontractor GFPM for the subcontractor to be able to more easily transfer its proposed FBLRs and DBA Insurance cost totals for each option period, to the prime offeror for the prime offeror to include in its proposal. |
| 2 | The FTEs and FBLRs (columns F & G) are automatically populated in this tab as they are pulled directly from the 'Subcontractor Labor Build' tab. |
| 3 | The DBA Insurance costs must be manually input by the Subcontractors into the yellow cells, if applicable. |
| "Total Compensation - Sub" Tab | |
| 1 | This tab is required for Cost Reimbursable Subcontractors and each Joint Venture Partner, Interdivisional, and/or Intra-Company entity, regardless of contract type. |
| 2 | Subcontractors for which this tab is required shall develop the Total Compensation (annualized based on Base Period start date) that an employee working on this effort would be expected to receive including Taxable Compensation, Social Security Tax, Medicare Tax, Unemployment, Workers Comp, Average Health & Welfare Costs per employee, average company contribution to Retirement Savings, Vacation, Holiday, Paid Time Off, and any other noteworthy benefits that contribute to the firm's ability to recruit and retain employees. |
