About this file
This is a First Article Waiver Worksheet (Revision C, dated 05 March 2014) used to request information and supporting documentation for waiving First Article Test (FAT) requirements on federal contracts.
The worksheet is designed for contractors to request either a Partial Waiver (specific tests only) or Full Waiver (all FAT testing) based on the Technical Data Package (TDP), contract, and solicitation requirements. Contractors must complete Section 1 with current acquisition information including the buyer's name, solicitation/contract number, contractor name and CAGE code, NSN, part number, top drawing number, noun, and revision level. Section 2 requires identification of the waiver type and specific TDP references for tests to be waived. Section 3 requires contractors to select and justify their waiver request using one of five justification categories: (A) successful FAT completion on a recent contract for the same item using identical facilities, equipment, processes, and subcontractors; (B) successful FAT on a similar item tested per the same requirements; (C) material in continuous production with documentation of the production run length and approval date of the last FAT; (D) successful FAT on a next higher assembly to eliminate duplicate testing on lower assemblies; or (E) other justifications with specific supporting documentation. Section 4 requires disclosure of any quality issues, manufacturing data changes, or Material Review Board actions. Contractors must attach all supporting documentation and submit the completed form on company letterhead signed by a company agent, including printed name, title, company name, address, and phone number. The form explicitly notes that partial waivers will not be accepted for this specific procurement and directs contractors to review Section E.2 of the solicitation for additional information.
