Field verification and identification of all storm drains within the project area is required, with potential clearing and grubbing within five feet of inlet/outlet areas. A qualified biological monitor meeting VSFB Front End Specification Section 01 57 20 must be provided and perform duties per AF Form 813. All work shall comply with National Association of Sewer Service Companies (NASSCO) Pipeline Assessment and Certification Program (PACP) standards. Contractor must conduct Closed-Circuit Television (CCTV) inspection of all storm drains, provide DVD/ROM copies of all video, and install government-approved Carsonite markers at all inlets and outlets following specified naming conventions. Deliverables include a comprehensive study report (within 14 business days of completion), GIS geospatial data updates, engineering drawings with elevation profiles and coordinates, condition ratings using PACP methodology, 100 CD/DVDs of CCTV videos, cost estimates in PACES and Unit Take-Off formats, and monthly invoices submitted electronically through WAWF. The contractor must develop and submit a Quality Control Plan addressing NASCCO, PACP, and OSHA standards prior to the pre-performance conference. Monthly progress meetings with government personnel will assess contractor performance against established thresholds, with potential remediation through Contract Discrepancy Reports for deficiencies. Hours of operation are 7:00 AM to 4:30 PM PDT Monday through Friday, excluding federal holidays. The contractor is responsible for providing all equipment, vehicles, tools, supplies, and personnel, while the government provides electrical and water utilities at job sites. These services are to be performed in accordance with industry standards, manufacturer specifications and Federal, State, and Local laws, statutes, and regulations. Cleaning and Survey of storm drain services are required to ensure all equipment listed in Appendix A will remain in continuous operation and all work is scheduled and completed in the service months as outlined in Appendix A. Contractor may utilize the previously performed study as a reference for reporting and naming convention. Previous project XUMU 18-1025B2 may be used as a reference as Appendix E, attached separately.
1.2 General Requirements
All work shall be accomplished in accordance with (IAW) all applicable codes, standards, laws, etc. that pertain to the effort. The survey shall utilize the National Association of Sewer Service Companies (NASSCO) Pipeline Assessment and Certification Program (PACP) for reporting.

The Services Summary (SS) and the Contractor’s Quality Control Plan provide information on contract requirements, the expected level of Contractor performance and the expected method of government validation and confirmation of services provided. These thresholds are critical to mission success.
Procedures set forth in RFO 52.212-4 (b), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.
Performance Objective PWS para. Performance Threshold
SS #1
Perform cleaning, videoing, and reporting of storm drains within the project area. See Appendix A (CLIN X001)
1.7 through 1.11
All cleaning, videoing, and GIS surveying is expected to be completed within 365 days.
2.1 Quality Control

The KO or COR will use the Contractor’s work schedule or a modified version thereof, to record validation results. The results of the validation then become the official Air Force record of the Contractor’s performance. When a performance threshold has not been met or Contractor performance has not been accomplished, the COR will initiate and provide the KO a Contract Discrepancy Report (CDR) DD Form 2772 for issuance to the Contractor. The Contractor shall respond to the CDR IAW instructions provided and return it to the KO within one (1) calendar day of receipt.

Failing to meet the performance threshold objectives for any two or more consecutive may constitute an immediate Progress Meeting with the Multi-functional Team.
2.3.2 Periodic Progress Meetings
The KO, Functional Commander, QAP, and other government personnel as appropriate, and the Contractor will meet once on the last Tuesday of every month during work performance period to discuss the Contractor’s performance.
