ATTACHMENT D - BIDDER RESPONSE TEMPLATE

DIVERSE BUSINESS INCLUSION PLAN - SUBCONTRACTORS (Mandatory Pass/Fail (M)) RFQQ 26-DCYF-CW-097 Stakeholder Engagement

Instructions: Bidders must complete this template and attach to their emailed RFQQ Response if they’re planning on utilizing Subcontractors to perform the Contract, if Awarded. Please thoroughly read all instructions in the RFQQ before completing.

ADMINISTRATIVE REQUIREMENTS

As set forth in the RFQQ (see section A.1), DCYF in accordance with Washington law and to the maximum extent practicable, encourages and supports small and diverse businesses to compete for and participate in state procurements as contractors and as subcontractors to awarded bidders.

For purposes of this RFQQ the following terms have the following meanings:

In support of the state’s economic goals and to support a diverse supplier pool, DCYF has established the following voluntary numerical goals for DCYF Competitive Solicitations:

· percent (10%) Office of Minority and Woman Owned Businesses;

· percent (1%) Veteran-Owned Businesses; and

· percent (20%) Washington Small Businesses, Achievement of these goals is encouraged whether directly or through subcontractors.

BIDDER’S ANTICIPATED DIVERSE BUSINESS PARTICIPATION (GOALS)

List Bidder’s anticipated small/diverse business category participation goals for Subcontractor participation, if bidder is awarded a Contract. Bidders may list any goal amount. These goals are aspirational goals for Bidder’s use of small/diverse business subcontractors, if awarded a Contract pursuant to this Competitive Solicitation.
