Bidders must confirm they are currently licensed to do business in Washington State or committed to obtaining licensure within 30 days of being announced as Apparently Successful Bidder, and must verify they have not been debarred or suspended from federal transactions. The template requests disclosure of any unsatisfactory contract performance or terminations in the past five years, current or recent State of Washington employment, and ability to provide required insurance coverage. Optional certifications for OMWBE, Veteran Owned Business, and Small Business status may be included, with bidders self-certifying as small businesses required to submit Attachment B.1.
