About this file
This is a Resident Bidder Affidavit required for the state of Kentucky in connection with RFP 758 2000000118 for Experimental Asian Carp Fishing Gear. The affidavit establishes the eligibility criteria for bidders claiming resident bidder status under Kentucky Revised Statutes 45A.494(2). Bidders must confirm under penalty of perjury that their entity is authorized to conduct business in the Commonwealth and has maintained continuous compliance with state requirements for the one-year period preceding the contract advertisement date. These requirements include filing Kentucky income taxes, making payments to the Kentucky unemployment insurance fund, and maintaining an active Kentucky workers' compensation policy throughout the specified timeframe.
The affidavit must be notarized and submitted as part of the bidding process, with the bidder providing printed name, title, company name, and address along with the notary's seal and commission expiration date. The bidding agency retains the right to request supporting documentation to verify any claimed resident bidder status, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. This affidavit serves as a mechanism to ensure that only qualified Kentucky residents or resident business entities participate in state contract opportunities.
