About this file

This is an Appendix B Quality Assurance Surveillance Plan (QASP) document that establishes performance standards and acceptable quality levels for contractor performance on a machine coolant maintenance contract.

The QASP defines three primary performance standards: (1) cleaning of additional machines added to Appendix A with zero tolerance for failures to respond to agreed arrival dates or perform unsuccessful cleaning, subject to 100% inspection by the Contracting Officer's Representative (COR); (2) continual maintenance of machine coolant with submission of clean-out data sheets within 10 calendar days of maintenance completion, allowing a maximum of two failures, subject to 100% COR inspection; and (3) tracking of coolant performance through laboratory analysis submission within 10 calendar days of coolant sampling (if Hocut 795 coolant is not utilized), also allowing a maximum of two failures, subject to 100% COR inspection. The document includes a Deliverables Schedule specifying two contractual data requirements: Clean-Out Data Sheet (CDRL A001) due within 10 calendar days after each machine maintenance with two electronic copies submitted to the COR, and Coolant Laboratory Analysis (CDRL A002) due within 10 calendar days after each coolant sample with two electronic copies submitted to the COR. Additionally, the plan includes a note requiring that forklift operators must always have a spotter with a whistle while in transit, per Naval Submarine Base (NNSY) Occupational Safety and Health and Environmental (OSHE) requirements. All contractor ratings are evaluated as either SAT (Satisfactory) or UNSAT (Unsatisfactory), with UNSAT ratings explained in the notes section.
