About this file
This document is an Attachment G - Responsibility Inquiry form for RFP S-10700-00013714 for Fire Sprinkler Testing, Maintenance and Repair issued by a state agency, likely in Oregon. The form is designed to comprehensively evaluate a proposer's qualifications, financial stability, past performance, legal standing, and capacity to successfully execute a fire sprinkler services contract.
The responsibility inquiry requires proposers to disclose detailed information about their business, including financial resources, contract history, legal challenges, outstanding judgments, bankruptcy history, licensing, and compliance with pay equity requirements. Key evaluation criteria include the proposer's ability to complete similar contracts within budget and time constraints, absence of significant legal or financial issues in the past three to five years, possession of required licenses and insurance, and financial capacity to fund daily operations throughout the proposed contract period. The form serves as a critical pre-award assessment mechanism to ensure only qualified and financially stable contractors are awarded the fire sprinkler testing and maintenance contract.
