About this file

This is a Past Performance Survey template used by the Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF) to evaluate contractor performance on completed federal contracts. The survey is designed to collect feedback from government contracting officers and project managers regarding a contractor's past performance across multiple evaluation categories. Completed surveys should be submitted directly to Brandon.Hodnett@atf.gov and Steven.Jaimes@atf.gov.

The survey evaluates contractor performance using a six-point rating scale ranging from Unsatisfactory (U) to Exceptional (E), with intermediate ratings of Marginal (M), Neutral (N), Satisfactory (S), and Very Good (VG). Performance is assessed across nine primary categories: Quality of Service, Timeliness of Performance, Business Relations, Management of Key Personnel, Compliance with Environmental, Safety, Health, and Security Requirements, and Contractual Considerations. Each category contains multiple specific evaluation criteria. The survey also requests narrative explanations for any ratings of Unsatisfactory or Marginal, and includes yes/no questions regarding contract violations, discrepancy reports, cure notices, option exercises, and whether the government would hire the contractor again. General information sections capture contractor name, contract number, dollar amount, period of performance, and respondent contact details.
