About this file

This is a Small Business Subcontracting Plan Review Checklist template used for evaluating contractor compliance with federal subcontracting requirements under solicitation FA4600-26-R-0002 for Offutt Air Force Base. The checklist is designed for the 55th Contracting Squadron (55CONS/PKT) to review individual small business subcontracting plans submitted by offerors, unless the contractor participates in the Comprehensive Subcontracting Plan Test Program under DFAR 219.702. The cumulative value of the basic contract and all options determines whether a subcontracting plan is required.

The checklist verifies 32 key requirements across four review periods: base period (12 months), plus four option periods and one six-month extension option. Reviewers must confirm that offerors establish subcontracting goals expressed as percentages of total contract value and subcontracted amounts for small business (SB) categories including Service-Disabled Veteran-Owned Small Businesses (SDVOSB), Women-Owned Small Businesses (WOSB), HUBZone, Historically Black Colleges and Universities/Minority Institutions (HBCU/MI), and Small Disadvantaged Businesses (SDB). The checklist requires verification of procedures for timely payment to SB subcontractors, identification of subcontracting types, methods for developing goals, identification of potential sources, inclusion of indirect costs, appointment of a plan administrator, efforts to ensure equitable competition among SB categories, and procedures for reporting through the Electronic Subcontracting Reporting System (eSRS). The checklist includes signature blocks for the Offeror, Procurement Contracting Officer (PCO), Small Business Specialist (SBS), and Small Business Administration (SBA) for approval or disapproval.

Checklist Individual Small Business Subcontracting Plan Checklist and Review
