About this file

This is a Schedule of Items pricing template for a firm fixed price (FFP) Request for Quotation (RFQ) issued by the Department of Agriculture Forest Service for pavement preservation work at the Vienna Parking Lot located at Hidden Springs Ranger District in Johnson County, Illinois. The solicitation number is 12445526Q0054 and is set-aside exclusively for small business concerns under NAICS code 237310 (Highway, Street, and Bridge Construction) with a size standard of $45,000,000. Contractors must provide unit pricing and total pricing for three line items: (406) Fog Seal at 53,000 square feet; (414) Cracks, Cleaning and Sealing at 1,500 linear feet; and (634) Pavement Markings Type K at 1,600 linear feet. All pricing must be entered in the schedule, and any work necessary to complete the project not listed is considered incidental and will not be compensated separately.

Key requirements for submission include that contractors must have active registration at sam.gov to be eligible for award (with "ID Assigned" status not meeting this requirement), and all terms and conditions of the solicitation are incorporated by reference through the contractor's signature on the offer. If the awarded contract exceeds $35,000, alternative payment protections/bonding will be required, with any bonding costs considered inclusive of the awarded price. Contractors with questions regarding missing or potentially additional schedule items should contact paul.boucher@usda.gov. Payment will only be issued for items explicitly listed in the schedule.

Schedule Notes:

1) One Firm Fixed Price (FFP) contract is anticipated from this RFQ. Contractor must enter pricing for all items.

3) Payment will only be made for the items shown in the schedule. Any work necessary to complete this project, that is not listed in the schedule of items, is considered incidental to the items shown. The preceding statement(s) do not authorize the contractors to insert new items into the schedule: if you believe an item is missing from, or should be added to, the schedule provided, contact paul.boucher@usda.gov.

4) Alternative Payment Protections will be required, if the awarded contract exceeds $35,000.00. All parties understand that the awarded price is inclusive of any incidental bonding costs; and the awarded price will not be modified to account for a contractor’s failure to include applicable bonding in their quote.
