About this file
This is an employee benefits document issued by the Unified Government of Athens-Clarke County outlining Term Life Insurance and Accidental Death & Dismemberment (AD&D) coverage options provided by Unum for full-time employees working at least 40 hours per week. The employer provides base Life and AD&D coverage equal to two times annual earnings, with a maximum of $500,000. Employees are eligible to purchase additional coverage ranging from $10,000 to $500,000 in $10,000 increments, subject to a combined maximum of $1,000,000 between base and additional amounts. Coverage is also available for spouses and dependent children under specified option tiers, with spouse coverage options ranging from $0 to $50,000 and dependent child coverage ranging from $0 to $15,000. The plan includes supplemental benefits such as a living benefit for terminal illness (up to 80% of the benefit or $500,000), waiver of premium provisions for total disability, and portability options for employees leaving the organization.
Coverage eligibility requires active employment at a U.S. business location, U.S. citizenship or legal work authorization, and absence from leave of absence or layoff status. The policy excludes suicide claims within 24 months of coverage effective date and contains specific AD&D exclusions for disease, war, riots, and intoxication-related incidents. Base and additional coverage amounts reduce to 45% of original amounts at age 75 and to 30% at age 80, with no subsequent increases permitted. Coverage terminates upon policy cancellation, loss of eligible group status, cessation of employment, or for dependents upon loss of eligibility, divorce, or death of the employee. The plan is underwritten by Unum Life Insurance Company of America and includes access to Life Planning Financial & Legal Resources services provided by HealthAdvocate.

Unified Government of Athens- Clarke County
Who can get Term Life coverage?
If you are actively at work at least 40 hours per week, you may apply for coverage for:
You: You can purchase additional Life and AD&D coverage from $10,000 to $500,000 in $10,000 increments, up to 10000 times your earnings.
If you previously purchased coverage, you can increase it up to $250,000, with no medical underwriting. If you previously declined coverage, you may have to answer some health questions.
The total amount of coverage between the base and additional amounts may not exceed $1,000,000.
Your spouse:
Option A $0 Option B $15,000 Option C $25,000 Option D or $50,000
Your children:
Option A $0 Option B $5,000 Option C $10,000 Option D $15,000
How does it work?
You choose the amount of coverage that’s right for you, and you keep coverage for a set period of time, or “term.” If you die during that term, the money can help your family pay for basic living expenses, final arrangements, tuition and more.
AD&D Insurance is also available, which pays a benefit if you survive an accident but have certain serious injuries. It pays an additional amount if you die from a covered accident.
Why is this coverage so valuable?
If you previously purchased coverage, you can increase it up to $250,000 to meet your growing needs — with no medical underwriting.
What else is included?
A ‘Living’ Benefit — If you are diagnosed with a terminal illness with less than 12 months to live, you can request 80% of your life insurance benefit (up to $500,000) while you are still living. This amount will be taken out of the death benefit, and may be taxable. These benefit payments may adversely affect the recipient’s eligibility for Medicaid or other government benefits or entitlements, and may be taxable.
Recipients should consult their tax attorney or advisor before utilizing living benefit payments.
Waiver of premium — Your cost may be waived if you are totally disabled for a period of time.
Portability — You may be able to keep coverage if you leave the company, retire or change the number of hours you work.
Employees or dependents who have a sickness or injury having a material effect on life expectancy at the time their group coverage ends are not eligible for portability.
