About this file
This document is a 2017 voluntary benefits proposal from Unum Group to Athens-Clarke County Government for whole life insurance coverage. The proposal presents a comprehensive voluntary benefits package designed to complement the county's existing health plan offerings, with an effective date of July 1, 2014, and coverage for approximately 1,700 eligible employees across a single class. The proposed whole life insurance product provides employees, spouses, and children with flexible coverage options ranging from $2,000 to $300,000 for employees, $2,000 to $75,000 for spouses, and $5,000 to $50,000 for children, with guaranteed level premiums and cash value accumulation. The plan includes optional riders such as accidental death benefits, long-term care coverage, and a child term rider, with enrollment conducted on a face-to-face basis at scheduled intervals. New employees face a 30-day waiting period, while current employees can enroll immediately with a minimum eligibility threshold of 20 hours per week.
The proposal structure emphasizes Unum's market position as a Fortune 500 company and leading provider of voluntary benefits, highlighting customer satisfaction ratings of 98 percent for likelihood of recommendation and 94 percent for overall value. Pricing is presented through detailed rate tables based on money purchase amounts ranging from $3.00 to $22.00 weekly, with separate rates for tobacco and non-tobacco users across various issue ages. The document includes comprehensive product descriptions, administrative support services, responsive claims processing within four days, and effective employee education and enrollment capabilities. Notable features include the Paid Up Option allowing premiums to cease at age 70 or 120, a guaranteed non-forfeiture interest rate of 4.5 percent, and tax-qualified long-term care riders (except in certain states). The proposal was valid until July 20, 2014, and contingent upon accurate employee information, timely payroll deduction administration, and minimum participation requirements.

 Future-ready benefit solutions from Unum
Today’s top trends — from the stalled economy to health care reform
— are having a strong impact on business. Offering Unum voluntary benefits can help you address these important challenges, while doing what’s best for your employees and your bottom line. It’s a winning strategy for today and the future.
Why include Unum voluntary benefits in your plan?
Partnering with Unum can help you balance the need to manage costs and provide financial protection for your employees. Our voluntary benefits include easy implementation, effective education and responsive customer service.
At the foundation of the Unum offering is a full range of competitive benefits solutions that appeal to a diverse workforce:
• Accident insurance
• Critical illness insurance
• Disability insurance
• Life insurance
At a time when most employees have little or no savings to fall back on if they face an illness or injury — and a record low number of
Americans have no life insurance at all — these coverages make perfect sense.
Voluntary benefits are budget neutral for the company and affordable for today’s employees — a refreshing twist for employers who are looking to add more coverage.
A reform-friendly solution
Many employers are concerned about the 40% “Cadillac” excise tax that will help fund health care reform.
Unum’s voluntary benefits are HIPAA-excepted* plans, so they are not subject to the Cadillac tax.
While they give employees more financial protection, they do not increase the actuarial value of your health plan under the current law.
*Critical illness insurance is exempt only when paid with post-tax dollars.

• Policy will mature at age 120.
• This option is available for issue ages 15-50.
• Rates are different based on Paid Up Option selected.

Family Coverage Options
Spouse Coverage
• The employee does not have to apply for coverage to purchase standalone spouse coverage.
• The spouse cannot be covered under both the spouse standalone policy and spouse term rider.
• The premiums are based on the issue age of the spouse.

Children's Whole Life Insurance Policy

• Coverage can be continued if the employee retires or leaves the company.
• This is available to children and grandchildren, regardless of dependency.
Children's Term Rider
• This is available to children, stepchildren and legally adopted children of the employee, who are unmarried, reside with the employee and are dependent on the employee for at least half of their financial support.
• This rider may be added to the employee or spouse policy, but not both. Employee or spouse must be 64 or younger.
• Annual premium is $6.00 per $1,000 of coverage.
• This rider must be added during an enrollment period when the first child is at least 14 days old in order for that child, and all future children, to have coverage.
• All future children are automatically covered after 14 days of age with no increase in premium.
• Death of the primary insured individual results in paid-up term coverage for each child until age 25.
• As each child reaches age 25, he or she may purchase whole life coverage, at current rates, for up to five times the amount of coverage inforce, subject to minimum policy requirements that apply to that contract.
The insured employee is responsible for notifying Unum in writing at least 31 days prior to the child’s 25th birthday if this change is desired.

NOTICE TO ERISA PLAN FIDUCIARIES: When ERISA governs, offering both employer paid and employee paid insurance products under a single ERISA plan and reporting them on a single Form 5500 gives maximum flexibility to employers to determine the proportion of employer and employee funding and to make decisions on how premium is allocated among coverages. The employer's payment obligation is the difference between the total cost of benefits and any employee contributions.
Proposal Conditions
ERISA Support: Unum supports treating all its group policies as employee welfare benefit plans governed by ERISA and will do so unless an employer advises that its plan qualifies for an exemption from ERISA. To assist employers in carrying out their ERISA responsibilities, employers who provide Unum with the necessary information can, in most cases, use Unum's certificate booklets to comply with their initial summary plan description disclosure requirements. Unum also provides employers with the premium and commission information needed to complete Schedule A on Form 5500 for group insurance. Similar information will be provided for other insurance policies, upon request. Please call 1-800-633-7491 for Broker Compensation Services to request such information.
Civil Union and Domestic Partners: Unum complies with all state civil union and domestic partner laws when applicable to our policies.

BROKER COMPENSATION DISCLOSURE NOTICE FOR GROUP PRODUCTS
Your insurance or benefits advisor can offer you advice and guidance as you select the policy and provider most appropriate for your needs. At Unum we recognize the important role these professionals play in the sale of our products and services and offer them a variety of compensation programs. Your advisor can provide you with information about these programs as well as those available from other providers. We support disclosure of broker compensation so that customers can make an informed buying decision.
Brokers may be eligible to receive Base Commissions and Supplemental Commissions from Unum.
Unless you have agreed in writing to compensate the broker differently, Unum provides Base Commissions to all brokers in connection with the sale of an insurance policy. Base Commissions are a fixed percentage of the policy premium, and may include a one time, first year flat amount for each policy sold. Base Commissions are paid by Unum to the broker(s) on your policy. In some circumstances, broker(s) may be eligible to receive commissions on your policy even after a broker of record change has occurred.
A broker may also qualify for Supplemental Commissions paid by Unum. For group insurance products, Supplemental Commissions may be paid as a fixed percentage of total eligible group insurance premiums.
• For New Sales premiums, the Supplemental Commission rate may range from 0% to 5.00% of total premiums paid. For certain group products, an additional 0% to 11.00% Supplemental Commission may be paid; and an additional flat amount per application may be paid for using our laptop enrollment system.
• For Renewal premiums, the Supplemental Commission rate may range from 0% to 2.00%.
The exact Supplemental Commission percentage payable to any broker is based upon the total dollar amount of all eligible inforce or new sales insurance premiums or number of policies that the broker had inforce with Unum in the prior calendar year. Supplemental Commissions may be calculated differently for other insurance products.
The premium you pay is not impacted whether or not your broker receives Supplemental Commissions.
If you would like additional information about the range of compensation programs our company offers for your group insurance policy or any other Unum insurance product, you can find more details at www.unum.com.
Should you have other questions not addressed by the website, including the Supplemental Commission percentage applicable to your broker, or if you want to speak to us directly about broker compensation, please call
