About this file

This is a Contractor Responsibility form (Attachment 2) for RFQ#FA520926Q0053 issued by the U.S. Air Force for furniture procurement to support SFS BDOC (Security Forces Squadron Base Defense Operations Center) facilities at Yokota Air Base, Japan, with both alternate and primary locations.

The form requires contractors to certify their financial resources and ability to meet delivery schedules, specify the completion timeline in months or days from contract award, and confirm they have provided model numbers, names, specifications, or catalogs for all items listed in the Statement of Work (Attachment 1). Contractors must verify that quoted prices account for installation services at Yokota Air Base Japan with off-base waste disposal through local Japanese municipalities, confirm whether subcontractors or joint ventures will be used, certify that prices are based on Direct Delivery Paid (DDP) shipment to the designated location with the contractor assuming all risk including damage, and affirm that all items offered are new and unused. The form serves as a template requiring contractor completion and certification by signature, with contractors obligated to notify the government immediately if any answers change prior to contract award.
