About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on July 20, 2017. The form serves as an informational guide for potential bidders to understand common requirements and potential disqualification issues in the bidding process for DEP construction projects. The document emphasizes that all bidders must thoroughly review the complete request for quotation and all related bid documents to identify specific project requirements and bidding procedures. The form does not establish individual project details such as quantities, response dates, site visits, or mandatory pre-bid meetings; rather, it functions as a general reference tool highlighting areas where bidders commonly encounter problems during the submission process.

The form identifies two categories of errors that may result in bid disqualification: immediate disqualification errors and errors that may be grounds for disqualification before contract award. Immediate disqualification factors include failure to attend mandatory pre-bid meetings, failure to sign required documents, lack of West Virginia contractor licensure, missing drug-free workplace affidavits, invalid bid bonds, non-acknowledgment of addenda when required, late submissions, federal or state debarment, and outstanding environmental enforcement actions. Secondary disqualification factors that must be resolved prior to award include uncontested state debt exceeding $1,000, workers' compensation or unemployment compensation delinquency, and failure to obtain required bonds and insurance. The form contains no pricing information, set-asides, funding sources, or contract term details, as it is designed solely as a reference checklist for bidders.
