About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The document serves as an informational guide for potential bidders on construction projects and outlines common bidding process problems rather than project-specific details. The form emphasizes that all potential bidders must thoroughly review the complete request for quotation (RFQ) documents, including all additional documents and instructions, as this checklist does not constitute a comprehensive list of all bidding requirements. The document explicitly states that failure to read and comply with all stated requirements in the bid documents may result in automatic bid disqualification.

The form identifies two categories of errors: those that result in immediate bid disqualification and those that may result in disqualification before contract award. Immediate disqualification errors include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to provide valid bid bonds, failure to meet any mandatory RFQ requirement, failure to acknowledge receipt of addenda if required, late bid submission, federal debarment, West Virginia state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Errors that may cause disqualification before award include outstanding debt to the state or political subdivision, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to provide a West Virginia contractor's license number within one day of request, failure to submit a signed drug-free workplace affidavit within one day of request, and failure to provide subcontractor listings within one business day of bid opening or DEP request.
