About this file

This is a Bid Opportunity List form template from the Wisconsin Department of Transportation (WisDOT) designed to comply with 49 Code of Federal Regulations Part 26.11 requirements for federal-assisted contracts. The form serves as a registry for all firms participating or attempting to participate in DOT-assisted contracts, including prime contractors, subcontractors, subconsultants, and vendors supplying materials and supplies. Prime contractors and consultants must complete all required columns when submitting bids and proposals, while subcontractors and subconsultants require prime contractors to complete identification and contact information plus any available information regarding firm characteristics. The form must be submitted with all bids, proposals, quotes, and expressions of interest in teaming arrangements on DOT-assisted projects.

The Bid Opportunity List collects standardized information from all bidding entities, including firm name, contact information (address, phone, email), Disadvantaged Business Enterprise (DBE) status, firm age, type of work performed, and annual gross receipts categorized into six brackets ranging from less than $500,000 to above $5 million. The form applies uniformly to both DBE and non-DBE firms, ensuring transparent documentation of all participants in the bid process for federal-assisted transportation projects. This requirement enables WisDOT to maintain comprehensive records of vendor participation and business diversity metrics on state and federal contracts.
