About this file

This is Amendment 1 to solicitation DOH26-003P issued by the Washington State Department of Health on December 12th, 2025, addressing vendor questions and updating contract terms for newborn screening cards. The contract supports the Office of Newborn Screening, which provides testing services for infants born in Washington, Hawaii, and Idaho using dried blood spot specimens collected on specialized medical filter paper cards. The solicitation includes five distinct card kits and related hearing screening test inserts. The contract term is one year with four renewal options available. The amendment clarifies that the ID Single card has been added to the bid pricing exhibit, establishes minimum order quantities for each card type, and specifies that pricing submissions should reflect costs for the first year only.

The amendment modifies pricing terms to allow annual price updates during the four renewal years, with adjustments calculated using the Consumer Price Index for Seattle-Tacoma-Bellevue data released in July, subject to DOH approval. All bidders must submit inclusive pricing that encompasses all cost elements without additional charges, regardless of payment method—whether cash, credit card, or electronic payment. Bidders must bear all processing or surcharge fees associated with credit card or electronic payment options. The updated Exhibit C Bid Price document, referenced as "Exhibit C Bid Price Amend," contains the minimum order quantities for each card and should be used for all bid pricing submissions. All other terms and conditions of the original solicitation remain unchanged.

PROJECT: DOH26-003P NEWBORN SCREENING CARDS

DATE: December 12th, 2025

TO: DOH26-003P NEWBORN SCREENING CARDS

FROM: Mariel Lizan, Procurement Coordinator DOH Contracts and Procurement

SUBJECT: Amendment No. 1– Answers to Vendor Questions, Update to solicitation, sections 4.3, Pricing and Exhibit C Bid Price

DOH amends DOH26-003P Newborn Screen Cards solicitation with answers to vendor question.

1. The Bid does not reference 'ID Single' card. Is that card excluded from the Bid?

No, the ID Single card has been added to Exhibit C, Bid Price.

2. Is there a MOQ (minimum order quantity) listed in the Bid anywhere for each card?

Not at this time, Exhibit C, Bid Price will be updated to include this information.

3. The bid is for 1 year, with 4 renewal options. Is the pricing to be submitted to be for 5 years? Or just the first year?

DOH requests that the price submitted be for the first year.

4.3 Pricing (Additional language has been added to support price changes based on CPI (consumer price index) for Seattle-Tacoma-Bellevue.) Section shall now read;

4.3 PRICING. Bid prices must include all cost components needed for the goods and/or services as described in this

Competitive Solicitation. See Exhibit C – Bid Price. A bidder’s failure to identify all costs in a manner consistent with the instructions in this Competitive Solicitation is sufficient grounds for disqualification.

• After year one (1) of the contract, prices may be updated annually for the four (4) remaining years of the contract. Price updates may only be calculated after the release of CPI (consumer price index for Seattle- Tacoma-Bellevue) data for the month of July. All price increases must be agreed to by DOH.

• Inclusive Pricing: Bidders must identify and include all cost elements in their pricing. In the event that bidder is awarded a Contract, the total price for the goods and/or services shall be bidder’s price as submitted.

Except as provided in the Contract, there shall be no additional costs of any kind.

• Credit Cards (P-Cards): In the event that bidder is awarded a Contract, the total price for the goods and/or services shall be the same regardless of whether Purchasers make payment by cash, credit card, or electronic payment. Bidder shall bear, in full, any processing or surcharge fees associated with the use of credit cards or electronic payment.

Exhibit C Bid Price has been updated with minimum quantities of order for each card noted on the bid price sheet.

New Exhibit C Bid Price has been added to documents on WEBS as Exhibit C Bid Price Amend. Please use this documents for all bid pricing.

All other terms and conditions in this Solicitation remain the same.
