About this file

This is an Affidavit of Eligibility Form for the Opportunity Zone Program administered by the Central Florida Tourism Oversight District in partnership with the State of Florida. The program aims to foster economic development and job creation in economically distressed communities by encouraging businesses, developers, and financial institutions to invest long-term capital in low-income census tracts. Eligible businesses must have their main office located within designated Qualified Opportunity Zones in Orange, Osceola, Lake, Polk, or Seminole counties. Businesses responding to solicitations may submit this affidavit to certify their eligibility for the program, even if not previously certified.

The form requires submission of business name, main office address, the designated opportunity zone county, and an authorized signature affirming that the main office location falls within the specified opportunity zone maps. Eligible businesses can reference the opportunity zones maps available through FloridaJobs.org and the IRS website to verify their location qualifies. This affidavit serves as documentation of eligibility for participation in state and local contract opportunities, such as the Rapid Infiltration Basins Rehabilitation project, that incorporate opportunity zone set-asides or preferences for economically distressed area businesses.
