About this file

This is an Affidavit of Eligibility Form for the Buy Local or Veteran Small Business Program applicable to state and local contract opportunities in Florida, specifically within the jurisdictional boundaries of Orange, Osceola, Lake, Polk, and Seminole Counties. The form serves as a certification document that bidders must submit with their bid responses for solicitations that include Buy Local or Veteran preference provisions. Businesses seeking to qualify under either program must complete the form by identifying their business type, providing their business name, and signing an affidavit certifying that they meet the established requirements. The form does not specify product or service requirements, as it functions as an eligibility verification instrument rather than a project-specific solicitation document.

Two distinct qualification pathways are outlined on the form. For the Buy Local program, businesses must have their headquarters, manufacturing facility, or principal office located within the five-county region for at least six months immediately prior to solicitation issuance, or the owner/principal must maintain primary residence within the jurisdictional boundaries. For the Veteran Small Business program, the business must be owned and controlled by veterans as defined in Section 502 of the Veterans Benefits, Health Care, and Information Technology Act of 2006, be domiciled in Florida, and be listed in the federal Database of Veteran-owned Businesses. Required attachments vary by program type and include a physical business address, a valid local business license tax receipt issued by the appropriate county, and additional verification documentation as requested by the district. Proof of veteran ownership and control status is mandated for veteran-focused applications.
